Özet

AMAÇ: Perkütan endoskopik gastrostomi (PEG); başta nörolojiknedenlerle olmak üzere; oral yolla beslenemeyen, ancak gastrointestinalsistem fonksiyonları sağlam olan hastalarda en sık tercih edilenbeslenme yöntemidir. Bu yazıda tek bir genel cerrahi uzmanının 90olguluk deneyimleri değerlendirilmektedir.YÖNTEM: Çalışmamızda, Otuz iki aylık süre zarfında ,PEGuygulaması gerçekleştirilen 92 hastanın kayıtları retrospektif olaraktarandı. Bu kayıtlar PEG endikasyonları, komplikasyonlar ve takipsüreleri açısından incelendi.BULGULAR: Doksan hastada pull PEG yöntemiyle işlem başarıylagerçekleştirilmişti. PEG işlemi gerçekleştirilen hastaların hiçbirindemajör komplikasyon görülmedi. Takiplerde en sık görülen minörkomplikasyon tüpün yerinden çıkması olarak belirlendi (n =11)Altı hastada (%6,6) PEG yerinde enfeksiyon gelişti. Takipte eksitusolan hastaların perkütan endoskopik gastrostomi ile ortalama takipsüresi 125 gün (5-466) olarak bulundu.TARTIŞMA: Perkütan endoskopik gastrostomi daha hızlı, dahagüvenli ve daha ucuz olması nedeniyle cerrahi gastrostomiyetercih edilir olmuştur. PEG işlemi düşük komplikasyon oranlarınasahiptir. Bu çalışma, PEG uygulamasının, üst gastrointestinalsistem endoskopisi eğitimi almış genel cerrahi uzmanlarınca, üstmerkezlere sevk edilmeden orta düzeydeki hastanelerde de iyisonuçlarla uygulanabileceğini göstermektedir.

Abstract

INTRODUCTION: Percutaneous endoscopic gastrostomy (PEG) is the most preferred method of delivering nutritional support in patients with a functional gastrointestinal system who can not be fed orally, most frequently due to neurologic reasons. This is the audit of 90 cases of PEG, performed by a single general surgeon. MATERIAL AND METHODS: The records of 92 patients who were considered for PEG placement during a 32 month period were retrospectively evaluated for the indications of PEG, complications, and the periods of follow-up. RESULTS: Percutaneous endoscopic gastrostomy was performed successfully in 90 ptients by the pull procedure. No major complications happened in these patients. The most frequently encountered minor complication was tube dislocation(n=11,%). The puncture site was infected in 6 patients(6.6%). In patients who died, the mean follow-up time after the tube placement was 125(5- 466) days. DISCUSSION: Percutaneous endoscopic gastrostomy has become the technique of choice for being a faster, safer, and cheaper procedure, limiting the surgical placement of gastrostomy tubes. It has low complication rates. This study shows that PEG can be performed with good results in secondary state hospitals by surgeons who were trained in upper gastrointestinal endoscopy, and these patiens do not have to be referred to tertiary centers.
