SURGICAL TREATMENT AND RADIOLOGICAL FINDINGS OF HYDATID CYST IN PATIENTS UNDER 16 YEARS OF AGE

Özet

AMAÇ: Bu çalışma cerrahi olarak son yedi yılda tedavi edilen pediatrik yaş grubundaki pulmoner kist hidatik hastalarının klinik, radyolojik, cerrahi tedavi ve takip bulgularını değerlendirdi. GEREÇ VE YÖNTEM: Kliniğimizde 2012 ile 2019 yılları arasında ameliyat edilen 38 pulmoner kist hidatik olgusunun demografik ve klinik verileri geriye dönük olarak incelendi. BULGULAR: Çalışmamıza dahil edilen 38 olgunun 24’ü erkek, 14’ü kadın olup ortalama yaş 11.9 yıl (1-16) olarak saptandı. Başvuru semptomu olarak olgularımızın 18’inde öksürük, altısında göğüs ağrısı, dördünde hemoptizi, dördünde hidatoptizi ve birinde hidropnömotoraks mevcuttu. Dört olgu başka bir sebeple çekilen direk grafi ile radyolojik olarak tanı aldı. Fizik muayene bulgusu olarak 19 olguda solunum seslerinde azalma, iki olguda ise ürtikeryal döküntü saptandı. Hastalığın görülme yerlerine bakıldığında 32 olguda sadece akciğer, beşinde akciğer ve karaciğer, bir olguda ise bilateral akciğer ve karaciğer tutulumu vardı. Dokuz olguda ise bilateral akciğer kist hidatiği görüldü. Cerrahi yaklaşım olarak 34 olguya unilateral, dört olguya farklı seanslarda bilateral posterolateral torakotomi ile kistotomi kapitonaj uygulandı. Unilateral posterolateral torakotomi uygulanan 34 olgunun üçüne akciğer kistotomi kapitonaj sonrası transdiyafragmatik olarak karaciğer kist hidatiği aspirasyonu ve kapitonajı uygulandı. Bir olguda ise akciğer cerrahisinden üç yıl önce dış merkezde laparatomi ile karaciğer kist hidatiği cerrahisi uygulanmıştı. Preoperatif olarak üç yıl süreyle albendazol tedavisi verilen bir olgu haricindeki tüm olgulara cerrahi tedavi sonrası albendazol ile medikal tedavi uygulandı. Bir olguda taburculuktan iki ay sonra kist kavitesinin açılması nedeniyle re-torakotomi ile yeniden kapitonaj yapıldı. İzlemde olguların hiçbirinde nüks saptanmadı. SONUÇ: Dünyanın pek çok bölgesinde pulmoner kist hidatik halen yaygın olarak görülmektedir. Serimizde tüm olgulara klinik ve radyolojik olarak tanı kondu ve hepsine torakotomi uygulandı. Ortalama takip süremiz 54.3 aydı ve olgularımızın tümü asemptomatik ve nükssüz seyretti. Pediatrik yaş grubunda görülen pulmoner kist hidatik hastalığında, kliniğe göre planlanmış cerrahi tedavi ve sonrasında medikal tedavi önemlidir

Abstract

AIM: This study was to evaluate the clinical, radiological, surgical treatment and follow-up findings of pulmonary hydatid cysts in the pediatric age group treated surgically in the last seven years. MATERIAL AND METHOD: Between 2012 and 2019, 38 pulmonary cyst hydatid disease patients who had undergone surgery in our clinic were studied retrospectively. RESULTS: Twenty four males and 14 females with a mean age of 11.9 (1-16) years enrolled. 18 of the patients had a couch, six had chest pain, four had expectoration of cystic contents. Four patients who admitted for another reason have diagnosed a hydatid cyst through a chest X-ray examination. 19 patients had decreased breath sounds, two had urticaria at physical examination. Cysts resided at one of the lungs in 32 patients, bilateral lungs in nine patients unilateral lung and liver in five, and bilateral lungs and liver in one patient. 39 patients had unilateral, and four had bilateral posterolateral thoracotomy and cystotomy capitonnage. Three of the 34 patients had transdiaphragmatic cyst aspiration and capitonnage of liver performed after lung cystotomy capitonnage. One patient underwent laparotomy liver cyst hydatid surgery three years before lung surgery. One patient underwent re-thoracotomy capitonnage due to cyst cavity reopening two months after the discharge. All patients except the one who had albendazole treatment were given postoperative albendazole. None of the patients had relapsed during the follow-ups. CONCLUSION: Pulmonary cyst hydatid is still a common disease in many regions of the world. All patients in our study were clinically and radiologically diagnosed and had a thoracotomy. The mean follow-up duration was 54.3 months. All the patients were asymptomatic, and none of the patients relapsed. This study confirmed that surgical treatment and following medical treatment according to patients' clinical state are critical in the pediatric age group who has pulmonary cyst hydatid disease.
