Tüberküloz olgularının radyolojik yaygınlık özelliklerine göre değerlendirilmesi

Özet

Radyolojik olarak ilerlemiş akciğer tüberkülozlu olgularda mortalitenin arttığı bilinmektedir. Çalışmamız ilerlemiş olguların özelliklerinin iyi belirlenmesinin tüberküloz tedavi yaklaşımına katkıları olabileceği düşünülerek planlandı. Çalışmaya aktif akciğer tüberkülozu tanısı almış, daha önce tedavi görmemiş yaş ortalaması 38.4$pm$15.1 olan 102 erkek olgu alındı. Olguların radyolojik yaygınlığı ile yaş, tanıda geçen süre, alışkanlıklar, ek hastalık, 1. ayda tedaviye yanıt ve ilaç rezistansı ilişkileri incelendi. Olguların 19'u (%18.6) minimal, 39'u (%38.2) orta, 44'ü (%43.1) ilerlemiş lezyonluydu. Tanılan için l aydan fazla süre geçen olguların çoğunluğu ilerlemiş lezyonluydu (p=0.011). Ayrıca ilerlemiş lezyonlularda ek hastalık (p=0.049) ve alkol kullanımının (p=0.028)daha fazla olduğu; 30 yaş üzerinde daha sık ilerlemiş lezyon olduğu görüldü (p=0.005). Minimal lezyonlularda radyolojik regresyon %84.2 iken, orta dereceli ve ilerlemiş lezyonlularda %64.1 ve %68.2 idi. Minimal lezyonlu olgularda 1 aylık tedavi sonrasında balgam ARB negatifliği %94.7, orta derecelilerde %87.2, ilerlemiş lezyonlularda %61.4 idi. Ek hastalığı. olanlarda daha az regresyon görülmesi de anlamlıydı (p=0.017). Öyküsünde alkol alışkanlığı ve ek hastalık olan, başvuruda geciken ilerlemiş tüberküloz olgularının da nüks eden kronik olgular gibi daha yakın takibe ve dikkatli tedaviye alınmaları gerektiği sonucuna varıldı.

Abstract

It is reported that the fatality is increasing among radiologically progressive tuberculosis cases. The main purpose of this study was to evaluate tuberculosis cases according to radiological features, which may influence the results of treatment of clinical tuberculosis. 102 males with a mean age of 38.4±15.1 years, active tuberculosis who never received any type of treatment for tuberculosis previously were included in our study. Other investigated study variables were radiological distribution of lesions, age, smoking and alcohol consumption, accompanying diseases, time spent between symptoms and diagnosis, response to treatment at the end of the first month and the clinical resistance to the treatment. 18.6% of cases had minimum, 38.2% had moderate, and 43.1% had progressive tuberculosis lesions. Most of the cases diagnosed more than one month after the onset of symptoms had progressive lesions (p=0.011). It was also determined that accompanying diseases (p=0.049) and alcohol intake (p=0.028) in progressive cases were significantly higher than others. Most of the progressive lesions were over age of 30 years old (p=0.005). According to radiological follow up, clinical regression among cases with minimum, moderate and progressive lesions were 84.2%, 64.1% and 68.1% respectively. After one-month treatment scheme the ratio of ARB negative cases among minimum, moderate and progressive groups were 94.7%, 87.1% and 61.4% respectively. Clinical regression among cases with accompanying diseases was significantly lower than others (p=0.017). As a conclusion; cases with smoking, alcohol consumption, and accompanying diseases as weir as cases with delayed diagnosis should be followed closely and treated as recurrent chronic cases.
