HCV enfeksiyonlu bir olguda liken planus ve psoriasis birlikteliği

Özet
Liken planus ve psoriasis vulgaris sık görülen inflamatuvar deri hastalıklarıdır, ancak aynı hastada nadiren birlikte görülürler. Her iki hastalıkla HCV infeksiyonu arasındaki muhtemel etyolojik ilişki ve inteferon tedavisine verdikleri yanıtlar hakkında çelişkili yayınlar mevcuttur. Bu bildiride, özgeçmişinde psoriasis vulgaris öyküsü olan diabetik bir kadın hastada, hepatit C ile birliktelik gösteren kutanöz ve oral liken planus olgusu sunuldu. İnterferon alfa-2a (IFN $alpha$-2a) tedavisiyle kutanöz liken planus lezyonları düzelirken oral lezyonlardan topikal siklosporin ve diğer topikal tedavilere direnç gösteren ağrılı erozyonlar gelişti. Ek olarak tırnaklarda, saçlı deride ve dizlerde psoriatik lezyonlar nüksetti. Olgu literatür bilgileriyle karşılaştırılarak tartışıldı.
Abstract
Lichen planus and psoriasis vulgaris are common inflammatory skin diseases, but they, rarely are found simultaneously in the same patient. There are several conflicting reports about the possible etiologic relationship between these two diseases and HCV infection, and, also, about their responses to interferon therapy. In this presentation, a diabetic, female case of cutaneous and oral lichen planus associated with hepatitis C, with a past history of psoriasis vulgaris was described. Cutaneous lesions of lichen planus were resolved under interferon alpha-2a (IFN $alpha$-2a) therapy, while oral lesions progressed to painful erosions, which were resistant to topical cyclosporin and other topical therapies. In addition, psoriatic lesions recurred on nails, scalp and knees. The case was discussed comparing with literature data.
