Özet

Kronik trahom ve kuru göz zemininde gelişen pterjiumun histopatolojik özellikleri ve trahomatöz kuru gözle birlikte olmayan pterjiumla karşılaştırılması amaçlanmıştır. Skatrizan trahom ve kuru göz (göz yaşı kırılma zamanı 5 sn veya altında ve Schirmer l test değeri 5mm veya altında) birlikte ligindeki 8 pterjium olgusu ile trahom ve kuru gözle birlikte olmayan 10 pterjium olgusunda eksizyonel doku örneklerinin histopatolojik incelemesi (formolle tespit, rutin doku takibi, 4 fim kesit alma, hemotoksilen eozin, Masson trikrom boyamaları, ışık mikroskopisi ve fotoğraflama) gerçekleştirilmiştir. Goblet hücre azalması, skuamöz metaplazi, epitelyal hiperplazi ve atrofı alanları, subkonjonktival elastotik dejenerasyon, fıbrosis, ödem, epitelyal hiperplazinin belirgin olduğu örneklerde psödopapiller yapılanmalar ve psödokriptik içe katlantılar ile iltihabi hücre infiltrasyonu her iki gruba ait örneklerde de saptanmış ancak trahom ve kuru gözün eşlik ettiği olgularda bu değişimler çok daha ağır olarak izlenmiştir. Yoğun lenfo-plazmositer hücre infiltrasyonu ve neovaskülarizasyon gelişimi sadece trahom ve kuru gözün eşlik ettiği pterjium olgularına ait örneklerde saptanmıştır. Trahomatöz skatrizasyon ve kuru göz varlığı pterjiuma ait olağan histopatolojik doku değişimlerini agrave etmekte ve tabloya kronik trahom ve kuru göze ait patolojiler eklenmektedir."

Abstract

The histopathological characteristics of pterygium associated with chronic trachoma and dry eye have been compared with those of pterygium without trachomatous dry eye. The excisional tissue specimens of 8 cases of pterygium with scarring trachoma and dry eye (tear film break-up time 5 seconds or lower, Schirmer I test value 5 mm or lower) and 10 cases of pterygium without trachoma and dry eye were histopathologically studied (fixation with formol, routine tissue following procedures; cross-section 4 p,m in thickness, staining with hematoxylin eosin and masson trichrome, light microscopy and photographing) Goblet cell loss, squamous metaplasia, epithelial hyperplasia and atrophy areas, subconjunctival elastotic degeneration, fibrosis, edema, pseudopapillary folding and pseudocryptic infolding in the tissue samples with apparent epithelial hyperplasia and inflammatory cell infiltration were determined in the specimens of both study groups; however, these changes were much significantly observed in the cases associated with trachoma and dry eye. Subconjunctival severe lympho-plasmocyter infiltration and neovascularization were observed only in the cases with trachomatous dry eye. Trachomatous scarring and dry eye aggravated the common histopathological tissue changes in pterygium and added some pathologic features of chronic trachoma and dry eye to common histopathological characteristics of pterygium.
