Kutanöz Allodinisi Olan ve Olmayan Migren Hastalarında Ağrı Şiddeti, Uyku Kalitesi ve Psikolojik Durumun Karşılaştırılması

Özet

Amaç: Bu çalışmada kutanöz allodinisi (KA) olan ve olmayan migren hastalarının ağrı şiddeti,uyku kalitesi ve psikolojik durumunu karşılaştırmak amaçlanmıştır.Gereç ve Yöntemler: Çalışma bir vaka kontrol çalışması olarak tasarlandı ve 32 migren hastası içerdi. Hastalar Allodini Semptom Kontrol Listesi’ne göre KA’sı olanlar (n=16) ve olmayanlar(n=16) olmak üzere iki gruba ayrıldı. Ağrı şiddeti Görsel Analog Skala (GAS) ile, uyku kalitesiPittsburgh Uyku Kalitesi İndeksi (PUKİ) ile, psikolojik durum Hastane Anksiyete (HAD-A) ve Depresyon Ölçeği (HAD-D) ile değerlendirildi.Bulgular: KA grubunda 6 (%37,5) hastada ciddi, 5 (%31,2) hastada orta, 5 (%31,2) hastada hafifderecede KA vardı. Kontrol grubunda 1 (%6,2) hastada hafif, 10 (%62,5) hastada orta, 5 (%31,2)hastada ciddi şiddette ağrı gözlendi. KA grubunda ise 8 (%50) hastada orta ve 8 (%50) hastada ciddi şiddette ağrı vardı. KA’lı bireylerin KA’lı olmayanlara göre GAS (p=0,005), HAD-A(p=0,034) ve PUKİ skorlarının (p=0,028) daha yüksek olduğu görüldü. İki grubun HAD-D skorlarıise benzerdi (p=0,089).Tartışma ve Sonuç: KA’sı olmayan migren hastalarına kıyasla, KA’lı migren hastalarında ağrı şiddeti ve anksiyete düzeylerinin daha yüksek olduğu ve uyku kalitesinin bozulduğu görüldü. KA’lımigren hastalarında tedavi planlanırken ağrı şiddetinin yanı sıra uyku kalitesinin ve psikolojikdurumun da göz önünde bulundurulması önemlidir

Abstract

Aim: In this study, we aimed to compare the pain severity, sleep quality, and psychological status in migraine patients with and without cutaneous allodynia (CA). Materials and Methods: The study was designed as a case-controlled study and included a total of 32 patients with migraine. Using the Allodynia Symptom Checklist, the patients were divided into two groups: those with CA (n=16) and without CA (n=16). Pain intensity and sleep quality were assessed with the Visual Analog Scale (VAS) and the Pittsburgh Sleep Quality Index (PSQI), respectively. Psychological status was assessed using the Hospital Anxiety (HAD-A) and Depression (HAD-D) Scale. Results: Of the patients with CA, 6 (37.5%) had severe, 5 (31.2%) had moderate, and 5 (31.2%) had mild CA. Of the controls without CA, 1 (6.2%) had mild, 10 (62.5%) had moderate, and 5 (31.2%) had severe pain. In the CA group, 8 (50%) patients had moderate and 8 (50%) patients had severe pain. It was observed that VAS (p=0.005), HAD-A (p=0.034), and PSQI (p=0.028) scores were higher in individuals with CA, while HAD-D scores were similar in the two groups (p=0.089). Discussion and Conclusion: It was observed that migraine patients with CA had higher levels of pain severity and anxiety and impaired sleep quality, compared to those without CA. In migraine patients with CA, it is important that treatment planning include evaluations of sleep quality and psychological status, as well as assessments of pain severity.
