Topikal uygulanan aprotinin ve traneksamik asitin açık kalp cerrahisi sonrası kan kaybına etkisi

Özet
Amaç: Açık kalp cerrahisi esnasındaki postoperatif kanamanın engellenmesinde topikal uygulanan aprotinin ve traneksamik asitin etkisin araştırmayı amaçladık. Yöntemler: Kardiyopulmoner baypas ile açık kalp cerrahisi yapılan 30 hasta 3 farklı gruba randomize edildi. Sternum kapatılmadan önce 1 gruba (n=10) 1000000 KIU aprotinin, 2. gruba (n=10) 2 gr. traneksamik asit ve 3. gruba (n=10) plasebo verildi. Ameliyat sonrası ilk üç saatte ki ve 24. saatteki toplam kan kaybı ve verilen kan ürünleri miktarı kaydedildi. Bulgular: Aprotinin alan grupta, postoperatif kan kaybı ilk üç saatte 122 ml, 24. saatte 302 ml ve toplam olarak 384 ml idi. Traneksamik asi verilen grupta postoperatif kanama ilk üç saatte 108 ml, 24, saatte 305 ml ve toplam olarak 393 ml idi. Kontrol grubunda ise sırasıyla 162,34; ve 502 ml idi. Transfüzyon ihtiyacı aprotinin grubunda 4.7 Ü, traneksamik asit grubunda 5.4 Ü ve kontrol grubunda 6 Ü idi. Sonuç: Aprotinin ve traneksamik asitin lokal uygulanması postoperatif kanamayı bir miktar azaltmaktadır ancak bu istatistiksel olarak anlamlı değildir. Aprotinin ve traneksamik asitin etkinliği arasında ise bir farklılık gözlenmemiştir. Ayrıca transfüzyon ihtiyacında da bir farklılık yoktu.
Abstract
Objective: We sought to investigate the effects of topical applications of aprotinin and tranexamic acid in preventing postoperative ble eding during open heart surgery. Methods: Thirty patients undergoing open heart surgery with cardiopulmonary bypass were randomized to three different groups. Grou| 1 ( n = 10) received 1000000 KIU aprotinin, Group 2 (n= 10) received 1 grtranexamic acid and Group 3 (n = 10) received placebo before do sure of the sternotomy. During the first three hours and 24 hours total postoperative blood loss and amount of transfused blood product were recorded. Results: In Group 1, postoperative bleeding rates were 122 ml during first 3 hours, 302 ml during 24 hours and 384 ml total. In Group 2, pos toperative bleeding rates were 108 ml during first 3 hours, 305 ml during 24 hours and 393 ml total. In control group, bleeding rates were 162,347 and 502 ml, respectively. Needs for transfusion were 4.7 U in the aprotinin group, 5.4 U in the tranexamic acid group and 6.0 U in the control group. Conclusion: Topical application of aprotininand tranexamic acid reduced postoperative bleeding, but this was not statistically significan The differences were insignificant for effectiveness between aprotinin and tranexamic acid and for transfusion requirements.
