İstanbul'da ası sonucu ölümlerin; 2000-2002 yıllarında yapılan otopsilerin retrospektif değerlendirilmesi

Özet
Ası, boynun bir bağ ile vücut ağırlığının indirekt etkisi ile sıkılması sonucu oluşur. Ası için, sıklıkla görülmekle birlikte her zaman tamamen dik bir pozisyonda bulunulması ve ayakların yerden ayrılması gerekmez. Ası, hükmi olanlar dışında çoğunlukla intihardır. Bazı olgularda ise kaza veya cinayet sonucu olur. Bu çalışmada İstanbul'daki Adli Tıp Kurumu Morg İhtisas Dairesinde 2000-2002 yıllarında otopsisi yapılan 486 ası sonucu ölüm olgusu değerlendirilmiştir. Bu amaçla Morg İhtisas Dairesi verileri retrospektif olarak değerlendirilerek, olgular yaş, cinsiyet, olay yeri, ası tipi ve ası vasıtası, hyoid kemik, tiroid kartilaj kırıkları, kan alkol ve diğer maddelerin düzeyi açısından incelenmiştir. Verilerin değerlendirmesinde SPSS 11.0 paket programı kullanılmıştır. Olguların 350'sinin (%72) erkek, 136'sının (%28) kadın olduğu, olguların yaş ortalamasının 37.03 (±16.1) olduğu, kadınlarda yaş ortalamasının 33.31 (± 16.15), erkeklerde ise 38.51 (±15.90), olguların %61.5'inde ası olayının meydana geldiği olay yerinin belirtilmediği, 425 (%87.4) olgunun tipik ası olduğu, 138 olguda (%28.4) hyoid kemikte kırık, 130 (%26.7) olguda thyroid kartilajda kırık ve 11 olgu hariç diğer tüm olgularda ekimoz bulunmuştur. 81 (%16.6) olgunun kanında etanol bulunmuştur. 29 (%6) olgunun kanında ise diğer maddeler saptanmıştır. Otopsi ve otopsi sonrası yapılacak toksikolojik, histopatolojik incelemeler her ası olgusunda ayırıcı tanı ve olayın ası ile intihar ya da öldükten sonra ası olayı olup olmadığının tespiti açısından önemlidir.
Abstract
Hanging is a constriction of the neck by a ligature, constricting force being applied indirectly to the ligature through the weight of the body. Hanging need not to be in the fully erect posture, with the feet clear of the ground, though this often happens. Excluding judicial execution, hanging is mostly suicidal, though some cases are accidental or homicide. In this study, 486 cases that performed autopsies in Morgue department of State Institute Forensic Medicine Istanbul because of hanging between 2000-2002 were evaluated. For this aim, Morgue department records were retrospectively investigated. Their ages, gender, crime scene, type of hanging and noose, injuries to the hyoid bone and the thyroid cartilage, level of blood alcohol and other drugs, material that used, were reviewed. Data were assessed with SPSS 11.0 package program. Of the victims, 72% were male and 28% were female. The average age of the victims were 37.03 (±16.1). The average ages of men and women were 38.51 ± (15.90) and 33.31 (±16.15), respectively. The crime scene was not reported in 61.5% of the cases. Of the cases, 87.4% were typical hanging. Hyoid bone fracture 28.4%, thyroid cartilage fracture 26.7% and except 11 cases bruises all of cases were reported. Of the cases in which the crime scene was known that the hanging occurred at the persons house in 26.2% of the cases. In 16.6% of victims, ethanol and 6% other drugs was detected in the blood. In every hanging case,autopsy, histopathological and toxicological examinations are important for distinguishing between suicidal hanging and the suspension of a body after death.
