Özet

Böcek sokmalarının neden oldu-ğu alerjik reaksiyonlar sıklıklaciddi tıbbi sorunlar oluştururlar.Böcek sokmaları içerisinde sıkgörülmesi ve önemi nedeniyleilk akla gelen arı sokmalarıdır.Arı sokmasına bağlı meydanagelen alerjik reaksiyonlar içerisindeen önemli olanı, en sıkmortalite nedeni olması dolayı-sıyla anaflaksidir. Arı sokmasınabağlı anafilaksinin sıklığı genelpopülasyonda %0.4 ile %5 arasındadeğişmektedir.Erzurum Adli Tıp ŞubeMüdürlüğü'nde 2008 yılında adlimuayenesi ve otopsisi yapılanarı sokmasına bağlı iki ölüm olgusudeğerlendirildi. Yaşları 48ve 50 olan iki erkek olgu, götü-rüldükleri acil servislerde kurtarılamamışlardır.Medikolegalotopsileri yapılan olgulardanbirinde total alt diş protezininfarenksi tıkadığı, diğer olgudaise mide içeriğinin trakea vebronşları tıkayarak asfiksi sonucuölüme neden olduğu tespitedilmiştir.Çalışma; anaflaksi tablosu ilehastaneye getirilen olgularda,hava yollarında mekanik tıkanmaoluşturabilecek nedenlerinde göz önünde bulundurulmasınıve bu durumun acil yardımesnasında anahtar rol oynayabileceğinivurgulamak amacı ileyapıldı.

Abstract

Allergic reactions related to insect bites usually cause major medical problems. Although many different types of insects in environment are able to inflict a poisonous bite or sting, hymenoptera stings comes to mind first because of the importance and frequency. The vast majority of serious medical problems and deaths result from anaphylactic reactions due to hymenoptera stings. Incidence of hymenoptera related anaphylaxis ranges between 0.4% and 5% in general population. In 2008, two hymenoptera stings related deaths were evaluated for which examination and autopsy were performed by the Directorate of Forensic Medicine at Erzurum. They were two males with the ages of 48 and 50, respectively. Autopsy findings of the first case showed that pharynx had been obstructed by the lower dental prosthesis. In the second case, the reason of death had been aspiration of stomach contents into the trachea and bronchus. These case reports suggest that, while treating anaphylaxis; it should always be considered that a mechanical obstruction of the airways may be present and an intact airway may play a key role in the first aid.
