Özet

Amaç: Çoğunluğuna kistotomi + kapitonaj tekniği ile operasyon uyguladığımız 60 hidatik kist olgusunu operasyon sonuçları ile birlikte gözden geçirdik. Materyal ve Metod: Ocak 1998 - Şubat 2000 arasında kliniğimizde 60 hidatik kist olgusuna operasyon uygulandı. Olgularda tanı klinik değerlendirme, akciğer grafisi ve toraks BT ile konuldu. Operasyon sonrası hastalara kontrendikasyon olmadığı sürece 1,5 ay süreyle 10 mg/kg/gün albendazol tedavisi uygulandı. Bulgular: 5 olguda hidatik kist bilateral, 11 olguda multipl, 3 olguda karaciğer kubbesi lokalizasyonundaydı. 29 olguda kist perfore iken 31 olguda intakttı. 55 hastaya kistotomi + kapitonaj uygulanırken, 2 olguya lobektomi uygulandı. Karaciğer kubbe hidatik kisti olan 3 olguya transdiafragmatik yolla kistotomi uygulandı. Seride erken ve geç ölüm oluşmadı. Postoperatif 3 olguda (%5) komplikasyon gelişti. Olguların hiçbirinde 2 yıllık izlemde nüks oluşmadı. Sonuç: Akciğer hidatik kist olgularında kistotomi + kapitonaj prosedürü ile rezeksiyon olmaksızın yeterli tedavi elde edilebileceği düşüncesindeyiz

Abstract

Background: We reviewed 60 hydatid cyst cases with results of operation whom most of them were operated by cystotomy and capitonnage technique. Methods: Sixty patients with hydatid cyst were operated in our clinic between January 1998 and February 2000. Diagnosis was made by clinical manifestations, chest x-ray and thorax CT. After the operation we treated the patient by 10 mg/kg/day albendazole for out 6 weeks if there were no contraindications. Results: In five patients the hydatid cysts were bilateral, in eleven patients the cysts were multiple and in three patients the cysts were at hepatic dome localization. Fifty-five patients underwent cystotomy and capitonnage, two patients underwent lobectomy. Three patients which the cysts were at hepatic dome localization underwent cystotomy via transdiafragmatically. There were no early or late mortalities. Three patients had postoperative complication. There were no recurrences in these cases for two years. Conclusion: We concluded that the procedure of cystotomy and capitonnage without resection is an efficient therapy in the pulmonary hydatid cyst cases.
