Özet

Yedi yıl önce ikili koroner bypass operasyonu geçirmiş ve son bir buçuk aydan beri anginal yakınması olan 61 yaşındaki erkek hastaya yaptığımız koroner anjiyografide safen ven greftte disseksiyon tespit ettik. İkili koroner bypassın birisi safen ven greft ile sağ koroner artere ve diğeri sol internal mammaryal arter ile sol ön inen artere idi. Hastanın safen ven greft anjiyogramında orta bölgede disseksiyon tespit edildi. Hastaya 2 hafta medikal tedavi uygulandı. Buna rağmen hastanın anginal yakınmalarında anlamlı bir iyileşme olmadı. Bu şartlarda, tekrarlanan koroner anjiyografide aynı disseksiyonun varlığı görülmesi üzerine, safen grefte önce balon anjiyoplasti ile predilatasyon yapıldı ve daha sonra 3.5 mm çapında ve 26 mm uzunluğunda stent implante edildi. Hastanın anginal yakınmaları hızla sonlandı.

Abstract

We report a 61 year old male patient with dissection of the saphenous vein graft on coronary angiogram who had undergone two vessels coronary artery bypass graft seven years ago, with left internal mammarial artery to left anterior descending coronary artery and saphenous vein graft to right coronary artery. For the last one and half month, he sustained angina pectoris. Coronary angiogram revealed a dissection of the saphenous vein graft at the mid-portion. His anginal symptoms couldn't be controlled despite maximal medical theraphy for two weeks. On repeat coronary angiogram, the dissection was found to be persisted and balloon angioplasty was performed for predilatation followed with implantation of a 3,5 mm diameter and 26 mm length Jo stent. Anginal symptoms of the patient rapidly resolved.
