İzole sol ön inen koroner arter lezyonunda stent ve çalışan kalpte koroner bypass operasyonu yöntemlerinin maliyet yönünden karşılaştırılması

Özet
Amaç: Bu çalışma, izole sol ön inen koroner arter (LAD) lezyonu olan hastalarda çalışan kalpte bypass (OPCAB) ve stent yöntemlerini ortalama prosedür maliyeti yönünden karşılaştırmak üzere yapıldı. Materyal ve Metod: Bir Kasım 2000 ile 30 Mart 2001 tarihleri arasında ardışık 17 hastaya OPCAB operasyonu (14 erkek, 3 kadın, ortalama yaş 55.6 ± 6.8 yıl), 30 hastaya ise stent uygulaması (23 erkek, 7 kadın, ortalama yaş 54 ± 7.9 yıl) yapıldı. Bulgular: Her iki hasta grubunda da mortalite görülmedi. Maliyeti etkileyebilecek perioperatif miyokard infarktüsü, revizyon, serebral olay, uzamış ventilasyon, intraaortik balon kullanımı ve aritmi komplikasyonlarından hiçbirisi OPCAB grubunda gelişmedi. Stent grubunda bir hastada işlem sırasında lezyon dilate edilemedi. Stent grubunda bir diğer hastaya stent sonrası tıkanıklık gelişmesi sebebiyle perfüzyonda koroner arter bypass operasyonu yapıldı. Genel hastane ve tek kullanımlık materyal maliyeti stent grubunda OPCAB grubuna göre daha yüksekti (p = 0.04). Transfüzyon maliyeti OPCAB grubunda stent grubuna göre daha yüksek bulundu (p = 0.039). Komplikasyon maliyeti açısından ise her iki grup arasında bir farklılık bulunmadı (p = 0.128). Üç ayrı başlığın birlikte değerlendirildiği ortalama prosedür maliyeti, stent grubunda OPCAB grubuna göre anlamlı derecede yüksek bulundu (1,821,353,000 ± 154,500,000 TL'ye karşılık 757,025,300 ± 142,213,800 TL; p < 0.0001). Sonuç: İzole LAD darlıklarında OPCAB operasyonu ortalama prosedür maliyeti yönünden stent yöntemine göre daha avantajlıdır.
Abstract
Background: This study was made to compare the average cost of the procedures in coronary artery bypass grafting on beating heart (OPCAB) and in that of stent implantation in patients with isolated stenosis of left anterior descending coronary artery (LAD). Methods: Between November 1, 2000 and March 30, 2001, 17 consequent patients underwent OPCAB operations (14 male, 3 women, the average age 55.6 ± 6.8 year) and stent implantations were performed in 30 patients (23 male, 7 women, the average age 54 ± 7.9 year) at our center. Results: There was no mortality in both groups. Any complications which may affect the total cost such as perioperative myocardial infarction, revision, cerebral accident, prolonged ventilation, intraaortic balloon pump insertion and arrhythmia were not observed in OPCAB group. Unsuccessful stent implantation was observed in one patient in stent group. Another patient was urgently operated with using cardiopulmonary bypass because of acute coronary occlusion in the stent group. The average cost of total hospital and nonreusable materials were higher in stent group than that of the OPCAB group (p = 0.04). The total cost of transfusion was significantly higher in OPCAB group comparing with in that of the stent group (p = 0.039). Regarding the cost of the complications both groups were similar. Conclusions: The total average cost of the procedures in OPCAB is much more advantageous than that of stent implantation for isolated stenosis of LAD.
