Serbest internal torasik arter greftlerinin proksimal anastomoz yerinin uzun dönem açık kalım oranına etkisi

Özet
Amaç: Serbest olarak kullanılan internal torasik arter (ITA) için klasik olarak tarif edilmiş olan proksimal anastomoz yeri direkt olarak aorta veya aortaya konulan bir perikard yamanın üzeridir. Ancak bunun mümkün olmadığı durumlarda proksimal anastomozu safen ven grefti veya in situ kullanılmış diğer ITA üzerine yapmak gerekebilir. Çalışmamızda proksimal anastomozu safen ven grefti veya in situ ITA üzerine yapılan serbest ITA greftlerinin uzun dönem sonuçları incelenmiştir. Materyal ve Metod: Proksimal anastomozu in situ ITA (Grup 1) veya safen ven grefti (Grup 2) üzerine yapılmış serbest ITA grefti kullanılan 84 hasta ortalama 38.1 ± 24 ay süreyle takip edildi ve bu hastaların 48 tanesine (%57.15) kontrol anjiyografisi yapılarak serbest ITA greftlerinin proksimal anastomoz yerinin uzun dönem açık kalım oranına etkisi araştırıldı. Bulgular: Kontrol anjiyografisi yapılan 48 hastanın 18'i Grup 1, 30'u Grup 2 hastası idi. Yapılan anjiyografiler sonucunda Grup 1'de 17 hastanın serbest ITA greftinin açık olduğu (%94.44); Grup 2'de ise 21 hastanın serbest ITA greftinin açık olduğu (%70) saptandı. İstatistiksel olarak gruplar arasında anlamlı bir fark saptanamasa da (p = 0.067), Grup 2 hastalarının serbest ITA greftlerinin anastomoz edildikleri safen ven greftlerinin kendisinde ve serbest ITA greftiyle anastomoz bölgesinde daha fazla probleme rastlanma olasılığı klinik olarak gözlemlendi. Sonuç: İnternal torasik arter greftinin in situ olarak kullanılamadığı durumlarda uzun dönem sonuçları safen ven greftlerinden daha iyi olan serbest ITA greftlerinin kullanılmasına çalışılmalıdır. Serbest greftin proksimal anastomozunun aort üzerine yapılmasının literatürde verilen sonuçları kabul edilebilir düzeydedir. Bunun mümkün olmadığı durumlarda, istatistiksel bir fark saptayamamakla birlikte, proksimal anastomoz yeri olarak in situ kullanılmış diğer ITA greftinin kullanıldığı olgularda açık kalım oranı daha yüksek bulunmuştur.
Abstract
Background: The usual proximal anastomosis site for free internal thoracic artery grafts (ITA) is the aorta or a pericardial patch layed on the aorta. A saphenous vein graft or the other in situ ITA should be considered in cases where the proximal anastomosis could not be done on the aorta. In the present paper, the influence of the proximal anastomosis site of free ITA grafts on the long-term outcome is studied. Methods: Eighty-four patients with free ITA grafts in whom the proximal anastomosis was on the other in situ ITA graft (Group 1) or on a saphenous vein (Group 2) were followed for a mean period of 38.1 ± 24 months. Fourty-eight patients (57.15 %) underwent control coronary angiography. Results: In Group 1, 17 of 18 (94.44%) angiographically controlled patients had patent free ITA grafts where this number was 21/30 (70%) in Group 2. The difference between the groups did not reach a statistical significance (p = 0.067) but we clinically observed that free ITA grafts in Group 2 patients were more prone to stenosis due to problems with their saphenous vein and with the anastomosis line between the vein and the free graft. Conclusion: Free ITA grafts should be considered before the saphenous vein grafts in cases where bilateral in situ ITA usage is not possible. The reported results of the proximal anastomosis on the aorta are acceptable for free grafts. In cases where this is not feasible, we have observed that the free grafts on the other in situ ITA has a better patency rate, even our results have bin not reach a statistical significance.
