Özet
Amaç: Göğüs duvarı tümörleri vücudun bütün primer tümörlerinin %2'sini oluşturan ve %60-80'i malign karakterde olan tümörlerdir. Cerrahi tedavide göğüs duvarı rekonstrüksiyonu da gerektirebilen geniş duvar rezeksiyonları yapılır. Materyal ve Metod: 1992 ve 2000 yılları arasında, kliniğinimizde primer göğüs duvarı tümörü tanısı ile opere edilen 36 olgu retrospektif olarak incelendi. Bulgular: Olgulardan 12'si kadın, 24'ü erkekti. Ortalama yaş 54'dü. Olgulardan %61'inde tümör kotlarda, %25'inde yumuşak dokuda, %14'ünde sternumda lokalize idi. En sık karşılaşılan malign tümör kondrosarkom (%36), benign tümör ise kondrom (%8) olarak saptandı. Rekonstrüksiyon amacı ile otojen kot grefti, kas flepleri ve sentetik greftler kullanıldı. Ortalama hastanede kalış süresi 7.6 gün olarak bulundu. Olgularımızda postoperatif mortalite görülmedi. Opere edilen malign karakterdeki 28 olgudan takip edilebilen 18 olguda 5 yıllık ortalama sürvi %45 olarak bulundu. Sonuç: Primer göğüs duvarı tümörlerinde doğru tedavi için erken teşhis ve uygun vakalarda yeterli rezeksiyon önemlidir.
Abstract
Background: Of all the primary tumors, chest wall tumors represent 2%, and 60 - 80% of those tumors have malignant characteristics. Wide resections of chest wall are performed which reconstruction may also be necessary for surgical treatment of chest wall tumors. Methods: We performed a retrospective study in 36 patients underwent surgery for the treatment of primary chest wall tumors at our clinic between 1992 and 2000. Results: 12 patients were females, 24 were males, mean age was 54. In our serie, 61% of tumor localizations were on the ribs, 25% of them in soft tissue, 14% of them were localized at sternum. Majority of the malignant tumors were chondrosarcomas. These represented 36% and benign tumors as chondromas comprised for 8% of all chest wall tumors. For reconstruction autologous costal grafts, muscular flaps and synthetic grafts were used in the surgical procedures.Average hospital stay was 7.6 days. No postoperative mortality occured within our serie. Mean five year survival rate was 45% in the 18 cases whom could be followed up out of 28 malignant cases. Conclusion: Early diagnosis and adequate resection is important for curative treatment in primary chest wall tumors.
