Özet

Enfektif endokarditin cerrahi tedavisinde homogreftler düşük enfeksiyon rekürrens oranları, hemodinamik ve fonksiyonel yönlerden de mükemmel sonuçları nedeniyle ilk tercih edilen konduitler olmakla beraber ülkemizde bunların elde edilmesindeki zorluklar nedeniyle yaygın olarak kullanılamamaktadır. Aort ve mitral kapak endokarditli hastamızda transplantasyona uygun olmayan donör kalbin aort kökünü ve pulmoner kökünü sırasıyla aort kök replasmanı ve mitral valv replasmanı için kullandık. Pulmoner kökün mitral konuma implantasyonu son yıllarda popüler olmuş ve henüz gelişme aşamasındaki bir cerrahi prosedürdür. Uyguladığımız tekniğin bu sürece katkıda bulunacağını düşünmekteyiz.

Abstract

Homografts are the first choice in the surgical treatment of infective endocarditis, because they have a low reinfection ratio and perfect functional and hemodynamic results. However because they are difficult to obtain, they are not commonly used in our country. We used antibiotic-preserved aortic and pulmonary homografts for aortic root replacement and mitral valve replacement respectively in the patient with native valve endocarditis. The implantation of the pulmonary homograft for mitral position is a new and recently popular technique and it is continuing to advance. This technique which we used in this case can contribute to this evolving process.
