Özet
Robot yardımı ile kalp cerrahisi önemli bir öğrenme eğrisi gerektiren farklı bir cerrahi yaklaşımdır. Kliniğimizde Nisan 2004 tarihinden itibaren kullanılmaya başlanan bu sistemle şu ana kadar 62 ameliyat gerçekleştirilmiştir. Bu ameliyatların üçü, ülkemizde bu alanda ilk kez gerçekleştirilen tam endoskopik koroner bypass ameliyatı idi. Üç olguda da cerrahi sadece portlardan, cerrahi kesi yapmadan gerçekleştirildi. Üç olguda da sol ön inen arterde darlık veya tıkanıklık vardı. Ameliyatta sol internal torasik arter sol ön inen artere anastomoz edildi. Ortalama anastomoz süresi 39 dk, kardiyopulmoner bypass süresi 71 dk, ameliyat süresi 281 dk idi. Ameliyat sonrası dönemde, bir olgu dördüncü saatte kanama sorunu nedeniyle revizyona alındı. Diğer iki olguda herhangi bir komplikasyon gelişmedi.
Abstract
Robotically enhanced cardiac surgery is a relatively new approach, with a steep learning curve. Sixty-two cardiac operations have been performed in our institution using the da Vinci robotic system since April 2004. Of these, three were totally endoscopic coronary artery bypass grafting, representing the first operations performed in this field in Turkey. Surgery was accomplished only through ports, without any surgical incisions. The patients had significant stenosis or occlusion of the left anterior descending (LAD) coronary artery. At surgery, the left internal thoracic coronary artery was anastomosed to the LAD artery. The mean durations of anastomosis, cardiopulmonary bypass, and operation were 39, 71, and 281 minutes, respectively. One patient required revision due to hemorrhage at the postoperative fourth hour. No complications occurred in the remaining patients.
