Özet

Edinsel arteriyovenöz fistüller, penetran travmalar sonucunda gelişebilir. Yetmiş iki yaşında erkek hasta nefes darlığı, bacakta şişlik ve ülsere yara nedeniyle kliniğimize yatırıldı. Kırk beş yıl önce ateşli silah yaralanması geçirmiş olan olgunun yapılan fizik muayenesinde konjestif kalp yetmezliği, sol bacakta venöz yetmezlik ve ülsere staz dermatiti olduğu saptandı. Bu olgu, travmatik arteriyovenöz fistüller hemen tanı konulup tedavi edilmezse belli bir zaman diliminde basit bir venöz yetmezlik kliniği ile konjestif kalp yetmezliği gibi kompleks yapıda karşımıza çıkabileceğini hatırlatmak amacıyla sunuldu.

Abstract

Acquired arteriovenous fistulas may have resulted from penetrant traumas. A 72-year-old male was hospitalized for dyspnea, swollen legs and ulserated dermatitis. Physical examination of the patient with a history of gunshot wound 45 years ago revealed congestive heart failure, left-leg venous insufficiency, and ulserated stasis dermatitis. We report this case, to remind that we can come across not just a simple venous insufficiency, but also a complex structure as congestive heart failure in a certain time period, if a patient with traumatic arteriovenous fistula is not diagnosed and treated immediately.
