Özet

Yapay greft ile infrarenal aort cerrahisi sonrası en korkulan komplikasyon greft enfeksiyonlarıdır. Kırk sekiz yaşında erkek hasta, geçirdiği infrarenal aortobifemoral bypass ameliyatından 17 ay sonra her iki femoral bölgeden akıntı yakınmasıyla başvurdu. Greft enfeksiyonu tanısı konularak yapay greft tümüyle çıkartıldı ve iki taraflı süperfisyel femoral venlerden oluşturulan otojen greftle yeni bir aortobifemoral bypass yapıldı. Ameliyat sonrası ikinci günde sol popliteal vende ve süperfisyel femoral venler distalinde akut tromboz gelişti. Düşük moleküler ağırlıklı heparine ek olarak, bacak elevasyonu ve elastik bandaj uygulamasıyla yakınmalar iki gün içinde geriledi. Ameliyat sonrası 10. günde taburcu edilen hastanın iki yıl sonraki kontrolünde iki taraflı femoral nabızların alınabildiği ve cerrahiye bağlı komplikasyon olmadığı görüldü.

Abstract

Graft infection is the most feared complication after prosthetic infrarenal aortic surgery. A 48-year-old male patient presented with leakage from both femoral sites 17 months after aortobifemoral prosthetic bypass grafting. With a diagnosis of graft infection, the prosthetic graft was totally removed and a new aortobifemoral bypass was performed using an autogenous graft prepared from both superficial femoral veins. On the second postoperative day, acute thrombosis developed in the left popliteal vein and in the distal part of the superficial femoral veins. The patients’ complaints subsided in two days following prophylaxis with low molecular weight heparin, leg elevation, and elastic bandage application. He was discharged on the tenth postoperative day. At the end of a twoyear follow-up, no surgical complications developed and femoral pulses were palpable on both sides.
