Bu yazıda santral alveoler hipoventilasyon tanısı konulan, meka- nik ventilatöre (MV) bağımlı bir hastada diyafragma pil uyarısisteminin (DPUS) etkinliği ortaya koyuldu. Solunum enfeksi- yonuna sekonder sepsis nedeni ile solunum yetmezliği oluşmuş65 yaşında erkek hasta MV ile yoğun bakımda takip edildi. Tümseptik bulgular düzelmesine rağmen solunum santral olaraktetiklenmediğinden hasta MVye bağımlı hale geldi. Türkiyedeilk kez laparoskopik cerrahi ile bir hastaya DPUS uygulandı.Hasta ameliyat sonrası ikinci günden itibaren DPUS kullanmayabaşladı ve dramatik bir şekilde MVden tamamen ayrıldı. Genelanestezi altında güvenli bir şekilde yapılan laparoskopik DPUSbenzer hastaların solunumsal ve diğer klinik durumlarını ve sağ - lıkla ilişkili yaşam kalitelerini iyileştirebilir. Abstract

This article aims to document the efficiency of diaphragmpacing stimulation system (DPSS) in a patient diagnosed withcentral alveolar hypoventilation and dependent to mechanicalventilation (MV). A 65-year-old male patient who developedrespiratory failure due to sepsis secondary to pulmonary infectionwas monitored in intensive care with MV. Although all septicsymptoms recovered, patient became dependant to MV sincerespiration was not triggered centrally. DPSS was implementedin a patient with laparoscopic surgery for the first time in Turkey.Patient started using DPSS as of postoperative second day andcompletely separated from MV dramatically. Laparoscopic DPSScan be performed safely under general anesthesia may improvesimilar patients respiratory and other clinic conditions as well ashealth related quality of life. Therefore, DPSS may be used morecommonly in patients with central alveolar hypoventilation.
