Is it safe to work in a fast-food kitchen? a case of recurrent spontaneous pneumothorax with cholesterol granuloma

Özet

Kolesterol granülomları, kolesterol kristalleri içeren,çevresi yabancı cisim dev hücreleri, fibröz doku reaksiyonuve kronik enflamasyon ile çevrili kistik lezyonlar olaraktanımlanmaktadır. Otuz bir yaşında erkek hasta solapikal nüks pnömotoraks yakınması ile göğüs cerrahisipolikliniğine başvurdu. Minitorakotomi ile çoklu kamarezeksiyon ve büllektomi yapıldı. Alınan materyalinhistopatolojik incelemesinde amfizematöz akciğer ve bül+ kolesterol granülomu tespit edildi. Hasta hazır gıdamutfağında, özellikle de patates kızartma bölümündeyaklaşık dört yıl süreyle çalışmış ve buharlaşan bitkiselve hayvansal kaynaklı eksojen yağ ve yağ ürünlerinesoluma yolu ile maruz kalmıştı. Hazır gıda mutfağında,özellikle patates kızartma bölümünde görev yapanpersonelin, maske kullanımı ve iyi bir havalandırmasistemi gibi koruyucu önlemler alınarak çalıştırılmalarıgerektiği kanaatindeyiz.

Abstract

Cholesterol granulomas are defined as cystic lesionscontaining cholesterol crystals surrounded by foreignbody giant cells, fibrous tissue reaction and chronicinflammation. A 31-year-old male patient presented atthe thoracic surgery outpatient clinic with a left apicalrecurrent pneumothorax. Bullectomy with multiple wedgeresections was performed through a mini-thoracotomy.Histopathological examination of the resection materialshowed emphysematous lung and bullae + cholesterolgranuloma. The patient had been working in a fast-foodkitchen and at the french fries section, in particular, fornearly the past four years and had been exposed throughinhalation to exogenous oil and oil products derivedfrom vaporized vegetable and animal oil. We believethat staff working at a fast food kitchen and at the frenchfries section, in particular, should work using protectivemeasures such as a mask and good ventilation.
