Özet
İnoperabl küçük hücreli dışı akciğer kanserinin konvansiyonel radyoterapi ile elde edilen tedavi sonuçlarının kötü olması farklı fraksiyonasyon şekilleriyle tedaviyi gündeme getirmiştir. Kliniğimizde Haziran 2000-Aralık 2001 tarihlerinde 30 hastaya konkomitant boost radyoterapi ile iki kolda, Co-60 ve linear akseleratörle tedavi uygulandı. Toplam 63 Gy doz (180 cGy/35 fx) uygulanarak tedavi 5 haftada tamamlandı. Tedavi sonunda 26 hasta değerlendirmeye alındı. Akut yan etkiler; cilt, özefagus ve akciğer toksisiteleri açısından değerlendirildi. Co-60 ile tedavi edilen l hastada grade 3 cilt toksisitesi haricinde başka hiçbir hastada grade 3-4 toksisite görülmedi. Sadece akut cilt reaksiyonları Co-60 ile tedaviye alman hasta grubunda linear akseleratörle tedavi edilen gruba göre istatistiksel olarak anlamlı bulundu (p=0.012). Radyolojik tam cevap oranı %7.6, parsiyel cevap oranı ise %50 olarak belirlendi. Sonuç olarak akselere konkomitant boostlu RT şemasının uygulanabilir ve akut toksisitenin kabul edilebilir olduğu ve iyi cevap oranları elde edildiği düşünüldü.
Abstract
Poor results obtained with conventional radiotherapy in the treatment of non-small cell lung cancer have revived treatment with different fractionations. In our clinic concomitant boost radiotherapy was applied to 30 patients between June 2000 and December 2001 in two arms namely with Co-60 and linear accelerator. Therapy was completed after total 63 Gy dose (180 cGy for 35 fractions) administered within 5 weeks. After completion of the therapy 26 patients were evaluated. They were assessed with respect to acute side effects, dermal, esophageal and lung toxicities. Among patients treated with Co-60, grade 3-4 toxicity was not seen except one patient with grade 3 dermal toxicity. Only acute dermal reactions were statistically significant in patients treated with Co-60 in comparison with the group treated with linear accelerator (p=0.012). Complete and partial radiological response rates were determined to be 7.6% and 50% respectively. In conclusion, it was thought that therapy schedule of accelerated concomitant boost radiotherapy is applicable with acceptable toxicities and improved response rates.
