Otolog Hematopoietik Kök Hücre Nakli Yapılan Multipl Miyelom Hastalarında Sağkalım Verileri

Özet

Amaç: Multipl Myelom (MM) hastalarında yüksek doz tedavi sonrası otolog hematopoietik kök hücre nakli (OHKHN) standart tedavi haline gelmiştir. Biz de merkezimizde MM tanısı ile takip edilen ve OHKHN uygulanan hastalarımızın genel sağkalım ve progresyonsuz sağkalım verilerini elde etmeyi amaçladık. Hastalar ve Yöntem: 2017-2020 yılları arasında İzmir Bozyaka Eğitim ve Araştırma Hastanesi, Hematoloji Kliniği takip ve tedavisi yapılan ve OHKHN uygulanmış MM tanılı 36 hasta yer aldı. Veriler geriye dönük dosya sistemi üzerinden tarandı. Bulgular: 36 MM tanılı hastadan 20’si erkek 16’sı kadındı. Medyan tanı yaşı 59 olarak saptandı. Hastaların tamamına başlangıç tedavisi olarak bortezomib içeren kemoterapi protokolleri verilmişti.ISS evreleme sistemine göre değerlendirildiğinde 6 hasta (% 16.6) ISS evre I, 16 hasta (%44.4) ISS evre II, 14 hasta (% 38.8) ISS evre III olarak değerlendirilmiştir. Mobilizasyon rejimi olarak 2 hastada yalnızca G-CSF (%5.5), 31 hastada Siklofosfomid ve G-CSF (%86.1), 3 hastada (%8.3) ise Siklofosfomid, G-CSF ve Pleriksafor kullanılmıştır. Hastalarımızın genel sağkalım oranı 1. yıl %85.0 ± 06.2, 2. yıl %74.5 ± 07.9 olarak bulunmuştur. Progresyonsuz sağkalım sonuçları ise 1. yıl %87.1 ± 6.1, 2. yıl %66.2 ± 9.5 olarak bulunmuştur. Sonuç: MM heterojen bir hastalıktır ve tedavi hasta özelliklerine göre bireyselleştirilmelidir. Tedavide etkin birçok farklı sınıfa ait yeni ilaçlar bulunmuştur. Özellikle hedefe yönelik tedaviler ile sağkalımda belirgin iyileşme elde edilmiştir. Mevcut verilerimize göre uygun hastalarda yüksek doz kemoterapi sonrası uygulanan OHKHN etkin bir tedavi seçeneğidir.

Abstract

Objective: Autologous hematopoietic stem cell transplantation (AHSCT) has become the Standard treatment after high-dose therapy in patients with Multiple Myeloma (MM). We aimed to obtain the overall survival and progression-free survival data of our patients who were followed up with the diagnosis of MM in our center and underwent AHSCT. Patients and Methods: Therewere 36 patients with a diagnosis of MM who were followed upand treated in the hematology department of the Health Sciences University İzmir Bozyaka Training and Research Hospital between 2017-2020 and underwent AHSCT. Data was scanned retrospectively through the file system. Results: Of the 36 MM patients, 20 were male and 16 were female. Median age at diagnosis was 59 years. Chemotherapy protocols containing bortezomib were given to all patients as initial therapy. When evaluated according to the ISS staging system, 6 patients (16.6%) were classified as ISS stage I, 16 patients (44.4%) as ISS stage II, and 14 patients (38.8%) as ISS stage III. As mobilization regimen, only G-CSF (5.5%) wasused in 2 patients, Cyclophosphomide and G-CSF (86.1%) in 31 patients, and Cyclophosphomide, G-CSF and Plerixafor in 3 patients (8.3%). The overall survival rate of our patients was found to be 85.0 ± 06.2% at the first year and 74.5 ± 07.9% at the second year. Progression-free survival results were found as 87.1 ± 6.1% at 1 year and 66.2 ± 9.5% at 2 years. Conclusion: MM is a heterogeneous disease and treatment should be individualized according to patient characteristics. New drugs belonging to many different classes have been found effective in the treatment. Significant improvement in survival has been achieved, especially with targeted therapies. According to our current data, AHSCT administered after high-dose chemotherapy in suitable patients is an effective treatment option.
