Özet
Amaç: Elektif sezaryenlerde oturur veya lateral pozisyonda uy- gulanan kombine spinal epidural (KSE) anestezinin maternal ve neonatal parametreler ile efedrin ihtiyacına etkilerini karşılaştıra- rak hangi pozisyonun hemodinamik ve teknik açıdan iyi olacağını göstermeyi amaçladık. Yöntemler: Altmış gebe oturur (Grup I, n=30) veya sağ lateral (Grup II, n=30) pozisyonda 10 mg hiperbarik bupivakain ve 20 μg fentanil ile KSE yapmak üzere rastgele iki gruba ayrıldı. Kalp hızı (KH), ortalama arter basıncı (OAB), duyusal ve motor blok özellikleri intratekal ilaç verildikten sonra operasyon bitimine ka- dar takip edildi. Efedrin ve ilk analjezik ihtiyacı, KSE için birden fazla deneme sayısı, spinal iğne girişine bağlı parestezi ve Apgar skorları kaydedildi. Bulgular: Efedrin ihtiyacında ve KH değişikliklerinde gruplar ara- sında fark bulunmadı. Ancak Grup IIde 45. dkdaki OAB, Grup Iden düşük bulundu (p<0,05). Grup IIdeki maksimum duyu bloğu seviyesi Grup Iden daha yüksekti (p<0,05). Motor blok gerileme süreleri iki grupta benzer olmasına rağmen, Grup IIde duyu bloğu gerilemesi ve ilk analjezik ihtiyacı Grup Iden daha geç gözlendi. Grup Ideki spinal iğneyle parestezi (Grup I=%3,3 ve Grup II=%20) ve birden fazla deneme sonucu KSE gerçekleştirme insidansı Grup IIden düşük bulundu (Grup I=%26,7 ve Grup II=%60) (p<0,05). Apgar skorları benzerdi. Sonuç: Sağ lateral pozisyonda duyu bloğunun daha hızlı ve daha yüksek seviyelere ulaşması ve KSE girişim sayısı ve spinal iğne giri- şine bağlı parestezinin daha fazla gözlenmesinden dolayı sezaryen- lerde gebeye KSE uygulamasının oturur pozisyonda daha güvenli ve kolay yapılacağı sonucuna varıldı.
Abstract
Objective: Our goal was to demonstrate which position would be hemodynamically and technically better by comparing the ef- fects of combined spinal epidural (CSE) in the sitting or lateral decubitus position for elective cesarean deliveries on maternal and neonatal parameters and ephedrine requirement. Methods: Sixty parturients were randomly assigned into two groups to perform CSE in the sitting (Group I, n=30) or right lat- eral decubitus position (Group II, n=30) using hyperbaric 10 mg bupivacaine and 20 μg fentanyl. Mean arterial pressure (MAP), heart rate (HR), and characteristics of sensory and motor block were recorded from intrathecal drug administration until the end of surgery. Ephedrine and 1st analgesic requirement, number of attempts to perform CSE, incidence of paresthesia during spinal needle insertion, and Apgar scores were recorded. Results: Ephedrine requirements and HR changes were similar in both groups. However, MAP values at 45 min in Group II were significantly less than in Group I. Maximum sensory block levels in Group II were significantly higher than in Group I. Despite similar motor block recovery times in both groups, regression times of sensory block and 1st analgesic requirement in Group II were significantly longer than in Group I. Incidence of paresthesia due to spinal needle (3.3% versus 20% in Groups I and II, respec- tively) and number of attempts to perform CSE (26.7% versus 60% in Groups I and II, respectively) were significantly higher in Group II. Apgar scores were similar. Conclusion: Performing CSE in the sitting position would be saf- er and easier because higher and earlier onset of sensory block, and a greater number attempts at epidural insertion and paresthesia develop to spinal needle insertion in the right lateral position.
