Özet

Helicobacter pylori (H. pylorij) enfeksiyonunun mide kanserinin en erken histolojik bulgularından biri kabul edilen epitel hücre proliferasyonu (EHP)'na da neden olduğu rapor edilmekte ve H. pylori eradikasyonu ile mide EHP'nda azalma gözlendiği bildirilmektedir. Bu çalışmada H.pylori pozitif midelerde EHP'nun oranını belirlemek ve eradikasyon tedavisi sonrasında EHP oranında gözlenen değişiklikleri araştırmak amaçlanmıştır. Çalışmaya dispeptik yakınmaları bulunan toplam 64 hasta dahil edilmiştir. Hastaların 44'ü H.pylori pozitif, 20'si ise H.pylori negatif grubu oluşturmuşlardır. Histopatolojik çalışmalar antrumdan alınan biyopsiler üzerinde yapılmıştır. EHPnu göstermek için immünohistokimyasal bir yöntem olan proliferating cell nuclear antigen labeling index (PCNA LI) kullanılmıştır. H.pylori pozitif olgularda PCNA LI oranı % 26.04 iken H.pylori negatif hastalarda % 19.81 saptanmıştır (p<0.05). H.pylori yoğunluğu ile PCNA LI arasında bir ilişki bulunmamıştır. Omeprazol ve amoksisilin'den iki haftalık tedavi ile eradikasyon oranı % 54.25 tespit edilmiştir. H.pylori eradikasyonunun sağlandığı olgularda PCNA LI oranlarında istatistiksel olarak anlamlı olan azalmalar gözlenmiştir. Bu sonuçlar, H.pylori enfeksiyonu olan hastalarda, EHP yüksek ise, eradikasyon tedavisinin faydalı olabileceğini düşündürmektedir. Böylece EHP'yi azaltmakla mide kanseri gelişiminin önüne geçilebilir. H.pylori pozitif mide mukozasında epitel hücresinin proliferatif cevabının hangi faktörlerin etkisi altında hangi neoplastik yöne doğru progresyon göstereceği açıklığa kavuşturulmamış sorunlardan biridir. Bu konuda geniş ve kapsamlı çalışmalara ihtiyaç vardır.

Abstract

Helicobacter pylori (H.pylori) causes gastric epithelial cell proliferation (ECP) which is known to be one of the earlier histological patterns of gastric carcinoma. Following H.pylori eradication therapy, a decrease in gastric ECP rate has been demonstrated. The objectives of this study were to determine the rate of ECP in H.pylori positive gastric mucosa, and to investigate the status of gastric epithelial proliferation following H.pylori eradication therapy. Sixty four patients with dyspeptic complaints were included in the study: 44 patients were H.pylori positive and 20 patients H.pylori negative. Biopsy specimens were taken from the antrum and evaluated, histopathologically. Proliferating cell nuclear antigen labeling index (PCNA LI), an immunohistopathological procedure, was used to show ECP rate. While the rate of PCNA LI in H.pylori positive patients was 26.04%, it was 19.81% in H.pylori negative patients. No correlation was found between the density of H.pylori and the rate of PCNA LI. The H.pylori eradication rate was 54.25% with a two week-com-bination therapy consisting of omeprazole and amoxycillin and in cases where eradication was achieved, the rate of PCNA LI significantly decreased. These results suggest that H.pylori eradication therapy can be useful if the rate of epithelial proliferation is increased in patients with H.pylori infection. Thus gastric cancer can be prevented by declining the rate of ECP. The question of which way the ECP will progress is unclear and further and extensive studies are necessary on this topic.
