Özet

Bilindiği gibi portal venin kavernöz transformasyonu olan olgularda biliyer sistemde en fazla koledokta olmak üzere sklerozan kolanjiti andıran bazı değişiklikler görülmektedir ve genellikle bu değişiklikler obstrüktif kolestaz bulgularına sebep olmamaktadır. Biz burada portai venin kavernöz transformasyonu ve buna bağlı portal hipertansiyonu olan ve obstrüktif sarılık bulguları gösteren dört olguyu sunuyoruz. Olgularımızda biliyer sistem ultrasonografi ve endoskopik retrograd kolanjiyopankreatografi (ERCP) ile incelenmiştir. Her dört olguda da intrahepatik safra yollarında dilatasyon, duvarlarında yer yer düzensizlik, koledokta segmentai daralma, ekstrensek bası (varise bağlı)saptanmıştır. Bu olgular koledoğa 7 F'lik plastik stent yerleştirilerek başarılı bir şekilde tedavi edilmiştir. Stentten sonra olgularda obstrüktif kolestaz bulguları klinik ve laboratuvar olarak tamamiyle düzelmiştir. Biz, böyle olgularda ERCP ile stent yerleştirilmesinin cerrahi girişimden önce düşünülmesi gereken bir tedavi yolu olduğunu düşünmekteyiz.

Abstract

Previous studies have found various changes resembling sclerosan cholangitis in the biliary system, mostly in common bile duct, in patients with cavernous transformation of the portal vein (CTPV). Generally, these changes do not cause obstructive cholestasis. We report four cases of portal hypertension and icterus due to CTPV. Patients were examined with ultrasonography and endoscopic retrograte cholangio pancreatography. In all four cases, we established dilatation of the intrahepatic bile ducts, segmentai narrowing in the common bile duct (CBD), extrensic compression due to varices and irregularities on the walls of bile ducts. The patients were succesfully treated with insertion of 7 F plastic stents into the CBD. Following the placement of stents, obstructive cholestatic symptoms resolved and laboratory findings improved. We conclude that endoscopic intervention should be considered prior to surgery in patients with bile duct narrowing due to CTPV.
