Özet

Toplumumuzda safra kesesi taşı olan hastalarda ursodeoksikolik asit (UDKA) tedavisinin etkinliğini, yan etkilerini araştırmak, tedavinin safra taşlarının doğal seyrine ve bu esnada gelişen komplikasyonlara etkisini, tedavi kesildikten sonra nüks oranı ve başarıyı etkileyen faktörleri belirlemek amacıyla yapılan çalışmaya minimal semptomatik olan 62 safra kesesi taşı olgusu alındı. 10 mm'den daha ufak taşı olan olguların % 58'inde taşlar eritilebilirken, 5 mm'den küçük taşı olan olguların tümünün taşları eritilebildi. Çapları 10 mm'den büyük ve multipl taşlarda erime oranı düşük bulundu (% 8.7). Hastalarda geçici ishal dışında önemli bir yan etki görülmedi. Tedavinin uzun sürmesi, ilacın maliyetinin yüksek olması ve taşların tedavi kesildikten sonra nüksetmesi bu tedavinin dezavantajlarıdır. UDKA ile tedavi, dikkatli hasta seçimi ile basan oranı büyük ölçüde artan, yan etkileri az olan, alternatif bir tedavi yöntemdir.

Abstract

In this study, the effectiveness of ursodeoxycholic acid (UDCA) therapy on the dissolution of gallstones, side effects, changes on the natural history of gallstones disease, complications, recurrence rates and factors which affects the succes rate were investigated. Sixty two minimally symptmatic patients with gallstones were included in the study. All stones which were smaller than 5 mm and 58% of stones smaller than 10 mm were successfuly dissolved. The succes rate in stones larger than 10 mm or multiple stones was low (8.7% ). There was no serious side effect except mild diarrhea. Disadvantages of this therapy are its long duration, high cost and high recurrence rate. Careful patient selection increases the success rate in this therapy. It has few side affects is safe, and may therefore be an alternative to surgical treatment in suitable cases.
