Özet
Duodenal ülser hastalığı ile kronik aktif antral gastrit arasında kuvvetli bir ilişki bulunmaktadır. Bu çalışmanın amacı, duodenal ülseri olan hastalarda tedavi sırasındaki gastrit tipi değişikliklerini izlemektir. Bu amaçla 125 duodenal ülserli hasta (74 E, 51 K; 37.75±11.35 yaş) çalışmaya alındı. Antrumdan alınan biyopsi spesmenleri mukozal inflamasyonu ve Helicobacter pylori (H. pylori) yi göstermek için sırasıyla hematoxylen-eosin ve Warthin-Starry boyası ile boyandı. H. pylori pozitifliğine göre anti-H. pylori tedavi veya H2 blokörü verilen hastalar 1, 2 ve 6 ay sonra kontrole çağrılarak endoskopik biopsiler tekrarlandı. Toplam 334 endoskopi yapıldı. Hastaların ilk endoskopilerinde kronik aktif süperfisyel gastrit (ASG) % 25.60, kronik nonaktif superfisyel gastrit (NASG) % 19.20, kronik aktif atrofik gastrit (AAG) % 36, kronik nonaktif atrofik gastrit (NAAG) % 19.20 ve H. pylori % 98.40 oranında bulundu. Tedavi sonunda aynı hastalarda ASG % 9.60, NASG % 12, AAG % 34.40, NAAG % 44 ve H. pylori % 52 bulundu. Tedavi başlangıcında aktif gastrit oranı % 61.60, superfisyel gastrit oranı % 44.80, atrofik gastrit oranı % 55.20 iken tedavi sonunda aktif gastrit oranı % 44, superfisyel gastrit oranı % 20.80, atrofik gastrit oranı ise % 79.20 bulundu. Aktif gastrit oranı tedavi ile belirgin olarak azaldı (p=0.02). Tedavi sırasında süperfisiyel gastrit tanısı azaldı, atrofik gastrit tanısı ise arttı (p<0.01). Süperfisiyel gastrit tanısının, tedavi sonunda atrofik gastrit olarak değişmesi, glandlardaki atrofinin yama tarzında dağılmasına veya daha düşük bir ihtimalle H. pylori etkisine bağlandı.
Abstract
There is a strong association between duodenal ulcer disease and chronic active antral gastritis. The aim of this study was to investigate the changes in degree of gastritis during the treatment of duodenal ulcer. We studied 125 patients with duodenal ulcer (74 M, 51 F; 37.75±11.35 yrs, m (SD). Biopsy specimens taken from the antrum were stained with hematoxylin-eosin and Warthin-Starry and examined for mucosal inflammation and H. pylori respectively. H2 receptor antagonists were given to H. pylori engative patients and anti-H. pylori treatment to the others. Investigations were repeated after one, two and six months and a total of 334 endoscopies were performed. In the first endoscopy, chronic active superficiel gastritis (ASG), chronic nonactive superficial gastritis (NASG), chronic active atrophic gastritis (AAG), chronic nonactive atrophic gastritis (NAAG) and H. pylori occurred in 25.60%, 19.20%, 36%, 19.20%, and 98.40%, of patients respectively. At the end of treatment, they occurred in 9.60%, 12%, 34.40%, 44% and 52% respectively. In the first endoscopy, while active gastritis, superficial gastritis, and atrophic gastritis occurred in 61.60%, 44.80%, and 55.20%, of patients, at the end of treatment they occurred in 44%, 20.80%, and 79.20% respectively. Active gastritis improved significantly with treatment (p=0.02). The rate of superficiel gastritis decreased and the rate of atrophic gastritis increased during treatment (p<0.01). The change of superficial gastritis to atrophic gastritis was attributed to patchy distribution of glandular atrophy or the effect of sustained H. pylori infection.
