Cystic dilatation of both extrahepatic bile ducts in a patient with primary sclerosing cholangitis

Özet
Primer sklerozan kolanjitin, kolonjiografik özelliği, kısa, anüler, multifokal darlıklar ve bu darlıkların arasında normal veya hafif dilate safra yollarıdır. Bugüne kadar sadece birkaç hastada intra veya ekstrahepatik safra yollarının kistik dilatasyonu bildirilmiştir. Bu yazıda 26 yaşında Crohn hastalığı ile birlikte sağ ve sol ekstrahepatik duktusların kistik dilatasyonuyla giden PSC'li bir olgu sunuldu. Ultrasonografide koledok duvarında belirgin kalınlaşma, ERCP'de ise diffüz daralmış koledok ve sağ (3.5X2.2 cm) ve sol (2.6X1.7 cm) ekstrahepatik safra yollarının kistik dilatasyonu görüldü. Intrahepatik safra yollarının normal görülmesi ve kontrast maddenin hızlı boşalması dilatasyonunun stenoza bağlı olmadığını düşündürdü. Ekstrahepatik safra yolunun kistik dilatasyonu PSC'nin nadir kolonjiografik bulguları arasında görülebilir ve ayırıcı tanıya ultrasonografi yardımcı olabilir.
Abstract
The cholangiographic features of primary sclerosing cholangitis are multiple strictures, usually short, annular and band-like, with intervening segments of normal or more usually mildly dilated ducts. Occasionally, cystic dilatations of intra- and extrahepatic bile ducts are observed in this disease. In this study, a 26-year-old male patient with Crohn's disease and extrahepatic primary sclerosing cholangitis with cystic dilatation of both hepatic bile duct is presented. Ultrasonography demonstrated a thickened common bile duct wall and endoscopic cholongiography showed a diffusely narrowed common bile duct and cystic dilation of the right and left extrahepatic bile ducts (3.5 cm x 2.2 cm and 2.6 cm x1.7 cm in diameter respectively). The normal (not dilated) appearance of the intrahepatic bile ducts and the rapid flow-of the contrast material from the intrahepatic bile ducts suggested that cystic dilatation was not of pre-stenotic origin in this patient. Cystic dilatation of both extrahepatic bile ducts may be an occasional radiological feature of primary sclerosing cholangitis and ultrasonography may be useful in differential diagnosis.
