Comparison of single and multiband systems in the management of esophageal varices

Özet
Amaç: Endoskopik bant ligasyonu kolay uygulanması, daha az komplikasyona ve hızlı varis eradikasyonuna yol açması nedeniyle endoskopik skleroterapiye tercih edilir. Tek bant ligasyonu (TBL) overtüp yerleştirmeyi gerektirir, bu rahatsız edici bir girişimdir ve komplikasyonları vardır. Bu nedenle birçok bant atabilen sistemler geliştirilmiştir. Bu çalışmanın amacı tek bant ligasyonu ve mültipl bant ligasyonunun etkinliğini karşılaştırmaktır. Yöntem: Benzer yaş, cins, child skoru, etyoloji ve ilk endoskopik bulguları olan 2 grup hastada TBL (n:147) ve mültipl bant ligasyonu (MBL) (n:37) uygulandı. Bulgular: Optimal varis eradikasyonu (OVE) TBL'nda 89.4 % ve MBL'nda 84.8% idi (p>0.05). Ortalama seans sayısı (1.96±1.07), optimal skleroz için seans başına kullanılan bant sayısı (6.39±3.12) MBL'nda TBL'ndan daha azdı (sırasıyla 3.65±2.21 ve 10.47±7.59) (p 0.05). MBL grubunda seans süresi daha kısa bulundu (3 dakika - 10 dakika) (p<0.01). Sonuç: MBL, TBL'una göre daha kısa sürer ve hızlı yapılır, optimal varis eradikasyonu daha az bant kullanılarak, daha kısa zamanda elde edilir.
Abstract
Background/Aims: Endoscopic band ligation is reported to be preferred to endoscopic sclerotherapy as it is easier to perform, has lower complication rates and allows quicker eradication of varices. The single band ligation (SBL) device requires overtube placement, is associated with discomfort and complications and is shielded from view. Multiple band ligation systems have therefore been developed for the treatment of esophageal varices. To compare the single band ligation (SBL) and multiple band ligation (MBL) systems developed for endoscopic band ligation. Methods: SBL (n: 147) and MBL (n: 37) were performed on two groups with similar age, gender, Child-P score, etiology, and initial endoscopic findings. The patients were compared with respect to various parameters. Results: Optimal varices eradication was 89.4 % in SBL and 84.8% in MBL (p>0.05). The median ligation session (1.96±1.07), and number of bands used per session (6.39±3.12) for optimal sclerosis were significantly lower in MBL than SBL (3.65±2.21 and 10.47±7.59 respectively) (p<0.01). The rate of variceal relapse, duration of relapse and early rebleeding were not different in the two groups: 25% vs 25.8%, 14.67±11.82 vs 10.0±4.86 months and 8.7% vs 3% respectively. It has been observed that the duration of the session was shorter in MBL group (3 minutes versus 10 minutes) (p<0.01). Conclusions: It was conluded that the application of MBL was easier and faster, and achieved optimal varices eradication in a shorter period of time with a lower number of bands compared to the SBL system.
