The effect of somatostatin infusion on plasma glucagon levels in cirrhotic patients with ascites

Özet

Amaç: Portal hipertansiyon olgularında somatostatinin portal basıncı düşürdüğü gösterilmiştir. Somatostatinin bu etkisinin artmış vazoaktif maddeleri azaltmasıyla ilgili olup olmadığı tartışmalıdır. Bu çalışmada asitli siroz hastalarında somatostatinin uygulanmasından sonra plasma glukagon düzeylerindeki değişikliklerin araştırılması amaçlanmıştır. Yöntem: Asidi olan 20 karaciğer hastası ve 20 sağlıklı kontrol çalışmaya alınmıştır. Gastrointestinal kanama, ensefalopati ve diabetes mellitusu olanlar ve son 2 hafta içinde düretik kullananlar çalışmaya katılmamıştır. Siroz hastalarına ilkgün kontrol peryodu olarak %5 Dextroz (ml/dk) 2 saat süreyle verildi, ikinci gün ise %5 dextroz (2ml/dk) içinde somatostatin (250 µg/saat) iki saat süreyle uygulandı. Kontrol ve çalışma peryodlarında, infüzyon sonrasında alınan kan numunelerinde plasma glukagon seviyeleri ölçüldü ve 20 sağlıklı kişinin plasma glukagon seviyeleri ile karşılaştırıldı. Bulgular: Asidi olan sirotik hastalarda kontrol peryodunda ölçülen ortalama plasma glukagon seviyesi (151 pg/ml), sağlıklı kontrol grubundan (78±15 pg/ml) yüksek bulundu. Aynı zamanda sirotik hastalarda somatostatin infüzyonu sonrasında ölçülen ortalama plasma glukagon seviyeleri (110±46 ml), kontrol peryoduna (151±58 ml) oranla belirgin olarak düşük bulundu. Sonuç: Asidi olan sirotik hastalarda plasma glukagon seviyeleri yüksek olup, somatostatin periferik kanda glukagon seviyelerini düşürmektedir. Somatostatinin portal kan akımını düşürücü etkisi bu vazoaktif hormon salınımını azaltması ile ilgili olabilir.

Abstract

Background/Aims: Somatostatin is an agent which decreases portal pressure and is widely used in cirrhosis for the treatment of acute variceal bleeding. However, it is still unclear whether this effect of somatostatin is due to the inhibition of several vasoactive substance that are increased in cirrhosis. Glucagon may mediate splanchnic vasodilation which contributes to portal hypertersion due to cirrhosis. The aim of this study was to determine the effects of somatostatin administration on plasma concentration of glucagon in cirrhotic patients with ascites. Methods: Twenty cirrhotic patients with ascites were included in the study. Those with gastrointestinal hemorrhage, hepatic encephalopathy, diabetes mellitus or having received diuretic therapy in the previous two weeks were not included. During the control period, 5% dextrose solution (2ml/min) was given by intravenous infusion to cirrhotic patients over a two hour period. Twentyfour hours after this period, somatostatin in %5 dextrose solution (2 ml/min) was given at a dose of 250 µg/hour for a two-hour study period. Plasma glucagon levels were measured in postinfusion blood samples of cirrhotic patients in both the control and study periods and compared with plasma glucagon levels of twenty healthy controls. Results: The mean plasma glucagon level in cirrhotic patients obtained during the control period was higher compared to healthy controls (151±58 vs 78±15 pg/ml respectively. There was also a significant decrease in the mean plasma glucagon level in cirrhotic patients following somatostatin infusion in comparison with the control period (from 151±58 or 110±46 pg/ml). Conclusions: Plasma glucagon levels are higher in cirrhotic patients. In addition, somatostatin can reduce peripheral blood glucagon levels and its portal pressure reducing effect may be related to a decrease in glucagon level.
