Özet

Endoskopik ve histopatolojik incelemeler, infla-matuar barsak hastalıklarında tanının konması ya¬nında, hastalığın yaygınlığının ve aktivitesinin saptan¬ması da, hastanın takibi ve tedavinin yönlen¬dirilmesinde en önemli yöntemler olmakla beraber zaman zaman birtakım kısıtlamalar ortaya çıkmakta¬dır. Biz bu çalışmamızda 13 ülseratif kolitisli hastada hastalığın aktivite ve yaygınlığının değerlendirilmesin¬de Tc 99m HMPAO işaretli lökosit sintigrafisinin değe¬rini, prospektif olarak inceledik. Sintigrafinin aktif barsak segmentlerini göstermedeki sensitivitesi %95, spesifitesi %96, pozitif tanımlama değeri %97, negatif tanımlama değeride %92 ve tanısal doğruluk oramda %95 olarak saptandı. Sintigrafik aktivite skoru; hem endoskopik, hem histopatolojik, hem de klinik aktivite skoru ile istatistiksel olarak anlamlı şekilde uyumlu bulundu (p<0.05). Sonuç olarak, Tc 99m HMPAO işa¬retli lökosit sintigrafisinin, ülseratif kolitisli hastalar¬da primer tanı aracı olan endoskopik ve histopatolojik metodların yerini tutmamakla beraber hastalığın akti¬vite ve yaygınlığını belirlemede, duyarlı ve güvenilir al¬ternatif bir yöntem olduğu kanaatine varıldı.

Abstract

Colonoscopy with multiple biopsies is the most accurate method for assessment of disease and ex¬tent in ulcerative colitis. However, it cannot be per¬formed every patients because it's some restrictions. In this study; scintigraphy with autologous leucaoytes la¬belled by Technetium 99m Hexamethyl pfopylene amine oxime (Tc 99m HMPAO) was performed in 13 patients with ulcerative colitis. In patients, scan activi¬ty score was compared with clinic, endoscopic and his-tologic activity score. Extent of disease in scintigraphy was compared with colonoscopy and biopsies. Sensitivi¬ty, specificity, positive and negative predictive value and over all diagnostic accuracy of scintigraphy in de¬tecting active inflammatory segments were 95%, 96%, 97%, 92% and 95%, respectively. Scan activity score was significantly correlated with clinic, endoscopic and histologic activity score (p<0.05). Tc 99m HMPAO leu¬cocytes scanning offer an accurate, non-invasive and safe alternative for assessment of disease activity and extent. But it is no substitute for endoscopic or histolog¬ic methods in the primary diagnosis, but it can help to direct the diagnostic strategy.
