Özet

Özofagus ve özofagogastrik bileşke karsinomalarının evrelendirilmesinde bilgisayarlı tomografinin doğruluğunu saptamak amacıyla 40 özofagus karsino-ma olgusunun bilgisayarlı tomografi bulguları, operasyon bulguları ve patoloji sonuçları ile karşılaştırıldı. Olguların bilgisayarlı tomografi incelemeleri lenf nodul metastazı, çevresel yaydım ve karaciğer metastazı yönünden değerlendirildi. Preoperaiif bilgisayarlı tomografi bulgularına göre olguların 26'sı operabl, 14'ü inoperabl kabul edilmiştir. Operabl kabul edilen olgulara küratif amaçlı operasyon uygulanmıştır. İnoperabl kabul edilen olguların yedisi palyasyon amacıyla öpere edilmiştir. Öpere edilen 33 olguda bilgisayarlı tomografinin sensitivitesi çevresel lenf nodul metastazı için %66, çevresel yaydım için %75, karaciğer metastazı için %80 bulunurken, spesifisite ve pozitif prediktif değer %100 bulundu. Negatif prediktif değer lenf nodul metastazı için %77, çevresel yaydım için %90, karaciğer metastazı için %97 bulundu. Doğruluk oranı lenf nodul metastazı ve çevresel yaydım için %91, karaciğer metastazı için %97 bulundu. Bu sonuçlar bilgisayarlı tomografinin özofagus ve özofagogastrik bileşke karsinomalarının evrelendirilmesinde kullanılabilecek doğruluk oranı yüksek bir inceleme yöntemi olduğunu ve inoperabl olgularda gereksiz operasyonu önleyebileceğini göstermektedir.

Abstract

In order to determine the accuracy of computed tomography in the staging ofesophageal and gastroesophageal junction carcinomas, computed tomography results were compared with operative and pathologic findings in 40 patients. Computed tomography examinations were evaluated with respect to nodal metastases, adjacent spread, and hepatic metastases. 26 of the cases were operable, 14 were inoperable according to the pre-operative computed tomography findings. Curative resection was performed to the 26 cases, and palliative surgery was performed to the seven inoperable cases. Sensitivity of computed tomography was 66% for nodal metastases, 75% for adjacent spread, 80% for hepatic metastases, whereas the specificity and positive predictive value were 100% for all three parameters for the 33 cases all of which underwent surgery. The negative predictive value was less than 77% for nodal metastases, 90% for adjacent spread and 96% for hepatic metasta¬ses. The accuracy were 91% for nodal metastases and adjacent spread, 97% for hepatic metastases. These results indicate that computed tomography is an accurate examination method for staging of the esophageal and gastroosephageal junction carcinomas and can prevent unnecessary surgery in patients with inoperabl tumors.
