Comparison of the effects of long term administration of cisapride and domperidone for the treatment of diabetic gastroparesis

Özet
Gastroparezili diabetiklerde uzun süreli cisapride ve domperidone kullanılımınm mide boşalması üzerin¬deki etkileri karşılaştırıldı. Mide boşalması, mideye ait yakınmalar, glisemik kontrolün derecesi çapraz ilaç kullanılımı şeklindeki bir düzenlemede karşılaştırıldı. Gastroparezisi olan 10 diabetik kişiye tesadüfi olarak ya 30 mg günde cisapride veya 60 mg günde domperidone en az 3 haftalık ilaçsız peryodlar bırakılarak 12 hafta süreyle verildi. Her bir ilaç rejiminin etkinliği semisolid işaretli bir gıdanın mideden boşalması, mideye ait ya¬kınmaların miktarı ve herbir tedavi ile sağlanan glise¬mik kontrol derecesi ile gösterildi. Mide boşalması (test yemeğinin %50 boşalması için gerekli süre) her iki ilaç¬la belirgin olarak hızlandı (p<0.05). Her bir ilaçla elde edilen sonuçlar diğerininki ile karşılaştırıldığında iki ilaç arasında belirgin bir fark yoktu. Açlık ve 2. saat postprandial kan glukoz seviyeleri gruplar arasında benzerdi. Rapor edilen gastrik yakınmalar her iki teda¬vinin sonunda belirgin olarak düzeldi (p<0.05). Gruplar arasındaki fark ise gene farklı değildi. Diabetik gastro¬parezi ile birlikte olan gastrik yakınmaları ve semisolid bir gıdanın boşalmasını düzeltmede cisapride veya domperidone kullanılmasının etkili olacağı tartışıldı. Her iki ilaç glisemik kontrolün düzelmesinde de etkili idiler.
Abstract
The effect of long term use of cisapride and dom¬peridone on gastric emptying in diabetics with gastropare¬sis were compared. Gastric emptying and symptoms as well as glycemic control were analysed in a crossover design. Specifically 10 diabetic subjects with gastroparesis were randomly assigned to either 30 mg of cisapride per day or 60 mg of domperidone daily for 12 weeks with a minimum 3 week washout period between the two drug treatment pe¬riods. The efficacy of each drug regimen was assessed by means of sequential gastric emptying scans assessing the emptying of a radiolabelled semisolid meal, gastric symptoms score and the degree of glycemic control achieved with each treatment. Gastric emptying (the time required for 50% emptying of the test meal) was significantly (p<0.05) accelerated by both drugs. Moreover, when the re¬sults achieved with each drug were compared to those of the other no significant difference between the two agents was evident. Glycosylated hemoglobin levels were reduced from a level of 9.8 ± 0.3% to 8.3 ± 0.1% with cisapride therapy (p<0.05) and from 9.7 ± 0.2% to 8.3 ± 0.2% with domperido¬ne therapy (p<0.05). Again, the results achieved with each agent were comparable. Fasting and 2 hour postprandial blood glucose levels were similar between groups. Reported gastric symptoms improved significantly at the end of both therapies (p<0.05). Again no difference between groups was evident. It can be concluded that the long term administra¬tion of either cisapride or domperidone to improve gastric emptying of a semisolid meal and to improve gastric symptoms associated with diabetic gastric paresis is suc¬cessful. Moreover an improvement in glycemic control oc¬curs also.
