A rare case of esophageal lymphangioma associated with gastric adenocarcinoma

Özet
Lenfanjioma; lenf damarlarının benign tümörüdür. Ge¬nellikle çocukluk çağında karşımıza çıkar. Bugüne kadar sade¬ce 7 adet özefagus yerleşimli lenfanjioma olgusu bildirilmiş olup, özefagial yerleşimli lenfanjioma ile mide adenokarsino-munun birlikteliği bildirilmemiştir. Bu makalede mide adeno¬karsinomu ile birliktelik gösteren özefagus yerleşimli bir len¬fanjioma olgusu sunulmuştur. Bu olgu zaman zaman ortaya çıkan karın ağrısı, yutma güçlü¬ğü, oral alımın olmaması, kilo kaybı ve genel durum bozuklu¬ğu ile yatırıldı. Kliniğe yatışını takiben yapılan üst gastrointes¬tinal sistem endoskopisinde dişlerden itibaren 32-34 cm'lerde lümeni tamamen kapatan kitle saptandı. Alınan biopsinin so¬nucu lenfangioma olarak rapor edildi. Hastanın tamamen dur¬muş olan beslenmesini sağlamak amacıyla cerrahi gastrostomi uygulandı. Gastrostomi yolundan beslenmede zorluk olması ve verilen gavaj mayiinin geri gelmesi üzerine gastrostomi ağzın¬dan baryum verilerek çekilen grâfılerde pilor kanalından dista-le geçiş olmadığı görüldü. Gastrostomiden fiberoskopla girildi. Mide içerisinde bol miktarda hemorajik mayii ve antrumu tutan kitle lezyonu görüldü. Buradan alınan biopsiler grade I adenokarsinoma olarak rapor edildi. Hasta bir hafta sonra kaybedildi.
Abstract
Lymphangioma is the benign tumour of lymphatic vessels. It is usually observed in childhood. Only 7 cases with esophageal localization have been reported until now, but neit¬her of them had been with gastric adenocarcinoma as the case reported below. The case was hospitalized with complaints of intermittent pain in the abdomen, dysphagia, no oral intake and weight loss. An esophagoscopy was performed to the patient and a mass that obstructed the entire esophageal lumen was observed at 32-34 cm from the teeth and multiple biopsies were taken. The biopsy material was reported as lymphangioma. Surgical gastrostomy was performed in order to feed the patient. When all feeding material came back, barium meal was given from the gastros¬tomy and X-ray studies showed that there was no passage thro¬ugh the pylorus. Profuse haemorrhagic material and a mass in the antrum was observed during endoscopy through the gast¬rostomy. Biopsies were, taken and reported as adenocarcinoma grade I. The patient died after a week.
