Helikobakter pilori tanısında kör mide biyopsisi

Özet
Helikobakter pilori(Hp) infeksiyonunun tanısının konul¬ması için, genellikle, mide mukoza örneği gerekmektedir. Sero-loji her ne kadar başlangıç tanısının konulmasında güvenilir bir metod ise de tedaviye yanıtın değerlendirilmesinde yararsız-dır.Nefes testi ise yaygın olarak kullanılmamaktadır. Gastrik mukozal doku elde etmek için dispeptik semptomları olan 33 hastaya(19 kadın, 14 erkek, ortanca yaş:38, sınırlar-.21-72) açık uçlu nazogastrik tüp (NGT) içinden önce kör mide biyopsisi (KMB) yapıldı. Takiben yapılan üst gastrointestinal endoskopi (ÜGE) ile biyopsi yerleri ve, varsa patolojik bulgular belirlen¬di. Ayrıca iki antrüm ve iki de korpusdan olmak üzere 4 adet endoskopik biyopsi(EB) alındı. Otuzüç olgunun 29'unda serum örneklerinden serolojik test de yapıldı. Tüm biyopsiler modifiye giemsa boyamasıyla hangi örneğin kime ait olduğundan ve hangi yöntemle alındığından habersiz olan bir patolog tarafın¬dan değerlendirildi. Endoskopik biyopsi ile alınan tedavi önce¬si örneklerde, 33 olgunun 31'de Hp+ idi. Bu 31 hastanın 30'un-da KMB'nde de Hp + bulundu. KMB ve kontrol endoskopisi, 14 gün, lBOOmglgün clarithromycine ve 28 gün, 40mg/gün omep-razol tedavisinin bitiminden 1 ay sonra 15 olguya tekrarlan¬dı. Onüç olguda her iki yöntemle de Hp- bulundu. Total olarak 48 KMB sonucundan 47'si EB ile uyumluydu. Kör mide biyop¬silerinin biri hariç hepsi korpusdandı. Tek uyumsuz sonuç ilk denemelerden birisine qitti ve örnek distal özofagusdan alın¬mıştı. Sonuç olarak KMB'nin Hp açısından değerlendirmek üzere mide mukozası elde etmede kolay, emniyetli, güvenilir, ve ucuz bir yöntem olduğunu söyleyebiliriz.
Abstract
Gastric mucosal tissue is usually required for the diagnosis of Helicobacter pylori(Hp) infection. Serology, though a reliable method in initial diagnosis, is useless in as¬sessing the response to the treatment. The breath test, on the other hand, is not widely available. We performed gastric mucosal biopsies via naso-gastric tube in 33 patients (19 fe¬male, 14 male, median age:38 ys, range:21-72) with dyspep¬tic symptoms. Blind gastric biopsy (BGB) was repeated in 15 Hp positive patients one month after completion of treatment with 500 mg tid Clarithromycin (14 days) and Omeprazole 20 mg bid (28 days). Upper gastro-intestinal (GI) endoscopy was performed immediately after BGB. The biopsy sites were noted and four other specimens, two from the corpus, and two from the antrum were taken. Serological testing was perfor¬med on the sera of 29/33 patients. All of the biopsies were evaluated by the same pathologist, with modified giemsa sta¬ining in blinded fashion. 31133 of initial biopsies obtained by endoscopic biopsy (EB) were positive for HP. BGB was posi¬tive in 30/31 of EB pozitives. After treatment, 13/15 (87%) were negative for Hp in both EB and BGB specimens and 2/15 (13%) were still pozitive. All but one of the BGB speci¬mens were from the corpus. In 47/48 (98%), BGB wds in ac¬cordance with EB. The only BGB sample with a result dis¬cordant from EB was from the distal esophagus, one of the first experiences. Serology was in accordance with biopsy re¬sults in all but one which was pozitive by histology and neg¬ative by serology. BGB was performed without anesthesia and tolerated well. We concluded that BGB is an easy, safe, reliable and cheap method in obtaining gastric mucosal tissue for Hp evaluati¬on. Its clinical value awaits further studies.
