Özet

Hepatit B virus pre-core ve core promoter mutas-yonlarının HBeAg negatif fenotiple ilişkili olabileceği bildirilmiştir. Bu mutasyonların prevalansı bir coğrafi bölgeden diğerine değişir. Bu çalışmanın amacı da kro¬nik hepatit B virus infeksiyonu olan Türk hastalarda pre-core ve core promoter mutasyonlarıhın prevalansını araştırmak ve bu mutasyonları hepatit B e antijen duru¬mu ile ilişkilendirmektir. Yirmi bir hastadan elde edilen serumda polimeraz zincir reaksiyonu ve direkt dizi ana¬lizi ile pre-core ve core promoter bölgeleri incelendi. Yir¬mi bir hastanın tümünde 1858. nükleotidde T mevcuttu. Pre-core stop kodon mutasyonu (A 1896), 11 hepatitis B e antijen pozitif hastanın 2'sinde (%18), 10 hepatitis B e antijen negatif hastanın 8'inde (%80) saptandı. Birinci T-A zengin bölgedeki değişiklikler (%40 hepatitis B e an¬tijen negatif hastada 1753. nükleotidde T den C'ye ve %30 hepatitis B antijen pozitif hastada 1752. nükleotid¬de A'dan C'ye) dışında hepatitis B e antijen pozitif ve ne¬gatif hastalar arasında core promoter mutasyonlarının gerek sıklık gerek' ortaya çıkma paterni açısından bir fark saptayamadık. Yalnızca bir hastada T 1762 ve A 1764'ün birlikteliği sözkonusu idi. Sonuç olarak kronik hepatit B'li Türk hastalarda hepatitis B e antijen negatif fenotip, pre-core mutasyonları ile ilişkili iken core pro¬moter mutasyonları ile benzer bir ilişki şüpheli görün¬mektedir.

Abstract

Mutations in the hepatitis B virus pre-core and core promoter regions have been reported to be asso¬ciated with hepatitis B e antigen negative phenotype. The prevalence of these mutations vary in different geograp¬hical areas. The aim of this study was to determine the prevalence of mutations in the hepatitis B virus pre-core and core promoter regions in Turkish patients with chro¬nic hepatitis B and to correlate presence of these mutati¬ons with, the hepatitis B e antigen status. Sera from 21 Turkish patients with chronic hepatitis B were analyzed by polymerase chain reaction amplifications and direct sequencing of the pre-core and core promoter regions. All the 21 patients had T at nucleotide 1858. The pre-core stop codon mutation (A 1896) was detected in 2 (18%) of 11 hepatitis B e antigen positive and 8 (80%) of 10 hepa¬titis B e antigen negative patients. Apart from changes involving the first T-A reach region (T-C at nucleotide 1753 in 40% hepatitis B e antigen negative and A-C at nucleotide 1752 in 30% hepatitis B e antigen positive pa¬tients), we found no difference in frequency or pattern of changes in the core promoter region between the hepati¬tis B e antigen positive and hepatitis B e antigen negati¬ve patients. Only one patient had co-occurrence of T1762 and A 1764. In conclusion, hepatitis B e antigen negative phenotype in Turkish patients with chronic hepatitis B is associated with mutations in the pre-core but probably not in the core promoter region.
