Karaciğer sirozlu hastalarda hepatik ve splenik arter akımlarının renkli Doppler sonografi ile değerlendirilmesi

Özet

Günümüzde, renkli Doppler sonografi portal hi¬pertansiyon tanısında, giderek artan sıklıkta kullanıl¬maktadır. Bu çalışmada, splenik ve hepatik arter çapı, akım hızı, hepatik arter sistolik akım hızının aortadaki sistolik akım hızına oranı, karaciğer sirozlu hastalar ile sağlıklı kontrollerde araştırıldı. Ayrıca karaciğer siroz¬lu hastalarda hepatik ve splenik arter çapları ve akım hızındaki değişiklikler ile sirozun etyolojisi, Child evre¬si, varis büyüklüğü arasındaki ilişkiler de araştırıldı. Çalışmaya 42 karaciğer sirozlu (28 erkek,yaş ort. 43.7+11.8 yıl) ve 9 sağlıklı kontrol (6 erkek, yaş ort. 48.7±9.5 yıl) alındı. Karaciğer sirozlu hastalarda kont¬rollere göre hepatik ve splenik arter çap (sırasıyla, 4.81+0.20 mm; p<0.001, 7.23±0.40 mm; p<0.008) ve akım hızları (sırasıyla, 303.6+37.68 cm/sn; p<0.001, 1260.5±162.9 cm/sn; p<0.05) anlamlı derecede yüksek bulundu. Hepatik arter sistolik akım hızının, aort sis¬tolik akım hızına oranının da anlamlı derecede arttığı saptandı(p<0.05). Ortalama hepatik arter çapı, Child C evresindeki karaciğer sirozlu hastalarda Child AveB' ye göre daha genişti (sırasıyla, p<0.01, p= 0.04). Ortala¬ma hepatik ve splenik arter çap ve akim hızlarının özo-fagus varis büyüklüğünün derecesi ile değişmediği, vi¬ral ve alkolik karaciğer sirozlu hastalar arasında da anlamlı derecede farklı olmadığı saptandı. Sonuç olarak; karaciğer sirozlu hastalardaki hepatik ve splenik arter hemodinamik değişikliklerinin, hepatik arter çapı dışındaki parametrelerde, karaciğer sirozu¬mun etyolojisi ve Child evresi ile ilişkili olmadığı, aynı zamanda özöfagus varislerinin derecesi ile de paralellik göstermediği saptandı.

Abstract

Nowadays, color Doppler sonography is used more often in the diagnosis of portal hypertension. In this study, the diameter of the splenic and hepatic ar¬teries, their blood flow, the hepatic arter systolic blood flow ratio to the aortic systolic blood flow was investiga¬ted in the healthy group and the patients with liver cirr¬hosis. In addition to these, the diameter of the splenic and hepatic arteries and the changes in their blood flow were compared according to the Child score, oesophage-al varices degree and the etiology of the liver cirrhosis. 42 patients with liver cirrhosis ( 28 male and 14 female, mean age 48,7±9,5) were investigated. In patients with liver cirrhosis the diameter of the hepatic and splenic ar¬teries (4.81±0.20 mm, p<0,001,7.23+0.40 mm;p< 0,008) and their blood flow (303.6+37.68 cm/sn; p<0,001, 1260.5+162.9 cm/sn; p<0,05 ) were significantly higher than controls. The ratio of the hepatic artery's systolic blood flow to the aortic systolic blood flow was also sig¬nificantly higher (p<0,05). The mean hepatic artery di¬ameter was greater in patients with Child C liver cirrho¬sis than in patients with Child A and B liver cirrhosis (p< 0.03, p< 0,01, respectively).The diameter and blood flow of the hepatic and splenic arteries did not change with the grades of the oesophageal varices and there was not a significant difference between the viral or alcoho¬lic liver cirrhosis. The increase in diameter and blood flow of the hepatic and splenic arteries in patients with liver cirrhosis did not correlate with the etiology of the cirrhosis, the Chil¬d's score and the degree of the oesophageal varices.
