Anti HCV pozitifliğinin tespitinde ikinci ve üçüncü jenerasyon ELISA testlerinin önemi

Özet
Hepatit C virusuna (HCV) bağlı hepatitler yüksek oranda kronikleşmeye eğilimli olmaları nedeni ile önemli bir sağlık problemidir. HCV infeksiyonunun varlığını tespit etmek için değişik sensitivite ve spesifite-lere sahip serolojik testler mevcuttur. Bu çalışmada 131 hemodiyaliz hastası ve 100 kan bankası donöründe ikinci ve üçüncü jenerasyon ELISA testler kullanılarak Anti HCV pozitifliği saptandı. Hemodiyaliz grubunda ikinci jenerasyon testle 66 (%50.4) hastada Anti-HCV pozitifliği saptandı. Üçüncü jenerasyon test aynı gruba uygulandığında 2. jenerasyonu negatif bulunan iki has¬tada Anti-HCV pozitif bulundu (üçüncü jenerasyonla Anti-HCV pozitiflik oranı %51.9 olarak saptandı). Kan bankası donörlerinde ikinci jenerasyonla hiçbir hastada Anti-HCV pozitifliği saptanmazken, üçüncü jenerasyon¬la bir kişide (%1) pozitiflik saptandı. İstatistiksel olarak ikinci ve üçüncü jenerasyon testler arasında farklılık gözlenmemesine rağmen üçüncü jenerasyon test bir mik¬tar daha sensitif olarak gözlenmektedir.
Abstract
Hepatitis C virus (HCV) infection is an im¬portant medical problem because of its tendency to chro¬nic hepatitis. Serological tests that are different in sen¬sitivity and specificity are available to detect antibodies against HCV antigens. In this study, second and third generation ELISA tests were compared to detect HCV antibodies in 131 hemodialysis patients in 100 blood do¬nors. HCV antibodies were positive in 66 (50.4 %) of the hemodialysis patients with the second generation tests. Two patients who were HCV antibody negative with the second generation tests were found to be HCV antibody positive with the third generation test. Total 68 (51.9%) hemodialysis patients were positive for anti-HCV with third generation test. There were no HCV antibody posi¬tive subjects with the second generation test in the he¬althy blood donors, whereas one (1%) was found HCV antibody positive with third generation test. We conclu¬de that the difference between two tests was not statisti¬cally important but third generation ELISA test seems to be more sensitive than its counterpart and further studies are needed to evaluate probable beneficial effect of third generation tests.
