Portal hipertansiyonda üst portosistemik şantların oral sintigrafik yöntemle değerlendirilmesi

Özet

Bu çalışmanın amacı sintigrafik süperiyor şant indeksi ile portal hipertansiyon komplikasyonları (özofagus varisleri, özofagus varis kanaması, portosistemik ensefalopati, asit) ve hepatosit fonksiyon belirleyicileri (serum albumin, protrombin zamanı, Child Pugh skoru) arasındaki ilişkinin ortaya konmasıdır. Çalışmaya siroz ve portal hipertansiyonlu 24 hasta ile 8 sağlıklı olgu alındı. Hasta ve kontrol grubundaki olgularda süperiyor şant indeksinin hesaplanması için ağızdan incebarsak pH'ında çözünen kapsüller içinde, 1mCi Tl-201 verildi. Olgulardan gamma kamera altında birer dakikalık 60 dinamik görüntü alındı. Oluşturulan zaman-aktivite eğrilerinden aktivitenin karaciğer ve kalpte ilk yükselmeye bağladığı andan 30 dakika sonraki değerler kullanılarak süperiyor şant indeksi (SŞİ) hesaplandı (SŞİ%=kalp(cnts)-karaciğer(cnts)/kalp(cnts) aktivitesi). Oral şant sintigrafisinden 4 gün sonra rektal şant sintigrafisi yapıldı. Rektal şant indeksi (RŞİ) ise gamma kamera dedektörü altına karaciğer ve kalbi alacak şekilde sırtüstü yatırılan hastanın rektum üst kesimine 10mCi Tc-99m perteknetat'ın nelaton sonda ile verilip karaciğer ve kalp üzerine ilgi alanları çizilerek elde edilen zaman-aktivite eğrilerinden 30 saniyedeki eğri alanları incelenerek hesaplandı (RŞİ%=kalp(cnts)-karaciğer (cnts)/kalp (cnts) aktivitesi). Hasta grubunda elde edilen üst portosistemik şant indeksi ortalama %38±2 iken bu değer kontrol grubunda %9±2 olarak bulundu (p<0.001). SŞİ ile portal hipertansiyon komplikasyonları ve RŞİ arasında anlamlı bir ilişki bulunmazken; protrombin zamanı ve Child Pugh skoru ile SŞİ arasında anlamlı ilişki bulundu (p<0.05). Sonuç olarak SŞİ'nin hepatosellüler fonksiyonlar açısından anlamlı bilgiler sağlayan bir parametre olduğu, fakat portal hipertansiyon komplikasyonlarının geliştiği hasta grubunun belirlenmesinde yeterli bilgiler vermediği kanısına varılmıştır.

Abstract

The object of this study was to establish the relationship between scintigraphic superior shunt index and portal hypertension complications as oesophageal varices, bleeding oesophageal varices, portosystemic encephalopathy, ascites and hepatocyte function indicators as serum albumin, protrombin time and Child Pugh's score. Our study included 24 patients with cirrhosis and portal hypertension and eight healthy subjects. 1mCi Tl-201 was given orally with enteric coated capsule and 60 dynamic images per minute were taken by gamma camera. After the first increase of activity in heart and liver with in 30 minutes, superior shunt index (SSI) was calculated (SSI%=heart(cnts)-liver(cnts)/heart(cnts) activity). Four days following per-oral shunt scintigraphy, rectal shunt scintigraphy was performed. This scintigraphic method was used by administering 10mCi Tc-99m pertechnetate in to the upper portion of the rectum via rectal tube. We calculated rectal shunt index (RSI) in the 30th second, recording regions of interest in the liver and the heart for 30 seconds using time activity curves (RSI%=heart(cnts)-liver(cnts)/heart(cnts) activity). While superior shunt index was approximately 38±2% in the patient group, the control group's value was 9±2% (p<0.001). Although there was no significant relationship between SSI, RSI and portal hypertension complications, a significant relationship was found between SSI, prothrombin time and Child Pugh's score. In conclusion, SSI was a parameter providing significant information about hepatocellular functions but inadequate data in determining patients with estasblished portal hypertension complications.
