The importance of endoscopic sphincterotomy in biliary fistulas secondary to hydatid disease of liver

Özet
Kist hidatik, dünyanın çeşitli bölgelerinde görülen ve sıklıkla karaciğeri tutan bir parazitik hastalıktır. Karaciğer kist hidatiğinin en sık görülen komplikasyonu safra yolları ile ilişkisinin olmasıdır. Büyük rüptürler operasyon öncesi kolaylıkla tanınıp cerrahi yolla tedavi edilmekle birlikte, daha küçük ilişkiler gözden kaçmakta ve postoperatif safra fistülü ile sonlanabilmektedir. Bu komplikasyonun tedavisinde endoskopik retrograd kolanjio pankreatografi ve sfinkterotominin (ERKP-s) cerrahiye alternatif olarak kullanılabileceği gösterilmiştir. Bu çalışmada da karaciğer kist hidatiği nedeniyle opere edilen ve postoperatif eksternal safra fistülü gelişen 9 hastaya ERKP-s yapılmış ve ek bir morbidite veya mortalite oluşmaksızın fistüller spontan olarak kapanmıştır. Endoskopik sfinkterotomi safra fistülü gelişen karaciğer kist hidatiklerinde ilk tedavi seçeneği olmalıdır.
Abstract
Hydatid disease of the liver is a parasitic infection and is endemic in many parts of the world. Communication with the biliary tree is the most frequent complication of hepatic hydatid disease. While frank ruptures are usually diagnosed preoperatively and dealt with surgically, simple openings may result in postoperative biliary fistula formation. ERCP and endoscopic sphincterotomy has proved to be a valid alternative to surgery in the treatment of this complication. Nine patients, who had been surgically treated for hepatic hydatid disease underwent ERCP for postoperative external biliary fistulas and all fistulas closed spontaneously without any additional morbidity or mortality following the procedure. We conclude that ERCP should be the first choice in the management of postoperative biliary fistula of hepatic hydatid disease.
