Biliyer sistem hastalıklarında safra ve serum karsinoembriyonik antijen (CEA) seviyeleri

Özet
Konvansiyonel yöntemlerle kesin tanı konamayan biliyer sistem kanserlerinde CEA'nin tanısal değeri olduğu tartışılmaktadır. Bu çalışmanın amacı,biliyer sistem hastalıklarında serum ve safra CEA düzeylerinin benign ve malign ayırımında tanıya bir katkısı olup olmadığını araştırmaktı. Kolanjiokarsinom tanısı konan (14'ü preoperatif, 7'si postoperatif) 21 hasta ile, benign safra yolu darlığı tanısı konan 17 hasta ve safra kesesi ve/veya safra yollarında taş tespit edilen 27 hasta çalışmaya alındı. Serum CEA seviyeleri radioimmunoassay, safra CEA düzeyleri ise enzim immunoassay yöntemi ile tayin edildi. Verilerin değerlendirilmesi sonucunda; serum CEA düzeyleri kanserli grupta (92.5 ± 65.8), benign darlıklı grupta(1.3 ± 0.6), taşlı grupta(1.6 ±1) idi. Kanserli grubun serum CEA değerleri; benign ve taşlı gruba göre anlamlı şekilde yüksek bulundu (P 0. 05). Safra CEA düzeyleri karşılaştırıldığında ise; kanserli grupta (9.2 ± 5), benign darlıklı grupta(1.3 ± 0.5), taşlı grupta (0.8 ± 0.5) idi. Kanserli grubun safra CEA değerleri; benign ve taşlı gruba göre anlamlı şekilde yüksek bulundu (P<0.0001). Benign grubun safra CEA değerlerindeki yükseklik taşlı grubun değerlerine göre anlamlı bulundu (P< 0.05). Kanserli olguların serum ve safra CEA seviyeleri arasında anlamlı bir pozitif korelasyon bulundu (r=0.716, p <0.0001). Sonuç olarak kolanjiokarsinomlu hastaların serum ve safra CEA seviyelerindeki yüksekliğin birbiriyle paralel olduğu ve safra CEA'deki yüksekliğin serum CEA'deki yükseklikte olduğu gibi tanısal değer taşıdığını, ancak safra CEA'deki yüksekliğin serum CEA'deki yüksekliğe üstün olmadığını ve malign ve benign safra yolu hastalıklarında serum ve safra CEA'nin ayırıcı tanıda kullanılabileceğini söyleyebiliriz.
Abstract
The diagnostic value of CEA in biliary tract carcinomas which can not be diagnosed by conventional methods remains controversial. The aim of this study was to investigate whether serum and bile CEA levels contribute to differentiation of benign and malignant disease in the biliary tract. Twenty one patients with cholangiocarcinoma, 17 patients with bening stricture and 27 patients with cholelithiasis were included in this study. Serum CEA levels were determined by radioimmunoassay, whereas bile CEA results were obtained by enzyme immunoassay method. Serum CEA levels were 92.5±65,8, 1.3±0.6 and 1.6±1 in cancer benign stricture and cholelithiasis patients respectively. Serum CEA levels in cancer patients were significantly higher when compared to those of the two other groups. There was no significant difference between the benign stricture and cholelıthiasis patients with respect to serum CEA levels. Bile CEA levels were 9.2±5, 1.3±0.5 and 0.8±0.5 in cancer, benign stricture and cholelithiasis patients respectively. Cancer patients had significantly higher bile CEA levels than those of the benign stricture and cholelithiasis groups. The benign group also had significantly bile CEA levels than those of cholelithiasis patients. A significant correlation was found between serum and bile CEA levels in cancer patients. In conclusion, serum and bile CEA levels are high and show a positive relationship with each other in patients with cholangio carcinoma. Both bile and serum levels have equal diagnostic value, and may be used in differentiating benign and malignant biliary tract disease.
