Peptik ülser operasyonlu hastalarda mide mukozasının akım-sitometrik analizi

Özet
Çalışmamızda en az 5 yıl önce gastrik veya duodenal ülser nedeni ile subtotal gastrektomi-gastrojejunostomi, vagotomi-piloroplasti uygulanan hastalarda zamana bağlı gastrik mukozal değişikliklerin akım-sitometrik analizi ile prekanseröz değişiklikler ortaya konulmaya çalışıldı. Yirmiyedi hastaya( E/K:25/2) 12 saatlik açlıktan sonra üst GİS endoskopisi yapıldı. Endoskopik değerlendirme sonrası anostomoz hattı veya antrumdan biopsiler alındı. Histopatolojik ve akım-sitometrik analiz yapıldı. Hastalar 4 gruba ayrıldı. Grup 1(n:4); ameliyat sonrası süre 10 yıldan az, grup 2(n:10);10-19 yıl, grup3(n:9); 20-29 yıl, grup 4(n:4); 30 yıl ve üzerinde. Histopatolojik değerlendirme: Grup 1: 4 vakada kronik nonspesifik iltihabi hücre inflitrasyonu (KNSİHİ), grup 2: 2 intestinal metaplazi, 3 atrofik gastrit, 5 kronik KNSİHİ, grup3: 4 atrofik gastrit, 1 normal, 4 KNSİHİ, grup4: 3 KNSİHİ, 1 displazi saptandı. Displazi saptanan hastada süre 34 yıldı. Akım-sitometrik analiz sonuçları: Proliferasyon İndeksi(PI): Grup 2 deki intestinal metaplazi grubunda ve grup 3'teki atrofik gastrit grubunda diğer gruplardaki histolojik değişikliklerden yüksek saptandı.(p<0,05) Grup 4 teki displazi ise tek vaka olduğu için istatistiki değerlendirilme yapılamadı. DNA indeksi: Tüm gruplarda histolojik dağılıma göre anlamlı fark göstermedi. Tek vaka olan displazide diğerlerinden oldukça fazla idi. Proliferasyon indeksi artmış olan displazili hastaya 34 yıl önce, grup 2' deki intestinal metaplazili 2, grup 3'teki atrofik gastritli 4 hastaya Billorth II ameliyatı yapılmıştı. DNA indeksinin benign olaylar nedeni ile mide ameliyatı geçirenlerde zamana bağlı olarak belirgin bir değişme göstermediği, PI'nin prekanseröz olabilecek histolojik değişikliklerde anormal hücre gelişiminin bir göstergesi olarak arttığı saptanmıştır. Atrofik gastrittede PI'nin zamanla arttığını bu nedenle operasyon sonrasında zamana bağlı olarak PI'nin histoloji ile korele olarak kanserin erken tanısı için kullanılabilecek bir belirleyici olabileceğini muhtemel kılmaktadır.
Abstract
The aim of this study was to find the presence of time dependent precancerous changes in the gastric mucosa of the patients who had undergone subtotal gastrectomy-gastrojejunostomy and pyloroplasty-vagotomy operation at least five years previously due to gastric or duodenal ulcer by flow-cytometric analysis. After a 12 hour fasting period, gastroduodenal endoscopy was performed on 27 patients included in the study, (M/F:25/2) and endoscopic evaluation was made. Biopsies from the antrum and anastomosis line were taken and histopathologic and flow-cytometric analyses were performed. Patients were separated into four subgroups In group 1(n:4) the period following surgery was less than 10 years, in group 2(n:10) this period was between 10-19 years, in group 3(n:9) the period was between 20-25 years and in group 4 (n:4) the period was more than 30 years. Results of histopathologic evaluation were as follow group 1: in four cases of elswere non specific inflammatory cell infiltration, group 2: Two cases of intestinal metaplasia, three cases of atrophic gastritis and five cases of chronic non-specific inflammatory cell infiltration, group 3: four cases of atrophic gastritis, in 1 normal case, and four cases of chronic non-specific inflammatory cell infiltration, group 4: three cases of non-specific inflammatory cell infiltration, and one case, (34 years after surgery), of dysplasia. Results of flow-cytometric analysis were as follows: Proliferation index (PI) In the intestinal metaplasia of group and in the atrophic gastritis group, PI was found to be significantly higher than the other groups (p<0.05). In the dysplasic group, the presence of one case made statistical analysis impossible DNA index: no diffrerence was found between the groups according to histological distribution. It was significant that the single case in the dysplasia group had undergone Billroth II surgery 34 years previously, dysplasia and a higher PI index was found in this patient. The two patients in whom intestinal metaplasia was found were in group 2 and they had also undergone Billroth II surgery as had the four patients in group III with atrophic gastritis. DNA index was shown not to have changed significantly in patients who had undergone surgery while PI was thought to be increased as a marker of abnormal cell development in possible precancerous histological changes. Atrophic gastritis, as a time dependent marker following surgery, can be used to detect early precancerous changes with correlation of PI and histology.
