Can the outcome of acute pancreatitis be improved in intensive care unit : A collaborative study

Özet

Amaç: Özellikle deneyimli personel kullanımı ve teknik olanakların iyileştirilmesi ile akut pankreatitin tedavisindeki gelişmeler mortalite oranlarını düşürmektedir. Yöntem: Bu çalışmadaki amacımız yalnızca genel cerrahların tedavi uyguladıkları Ankara Numune Hastanesi Genel Cerrahi Yoğun Bakım Ünitesindeki akut pankreatitli 50 hasta ile, dahiliye uzmanı, genel cerrah ve yoğun bakım uzmanlarından oluşan bir ekip tarafından multidisipliner tedavi uygulanan Pittsburgh Üniversitesi Sağlık Merkezinin Yoğun Bakım Ünitesindeki akut pankreatitli 52 hastanın sonuçlarını karşılaştırmaktı. Bulgular: ANH'deki hastaların %70'inde (35 hasta), UPMC'daki hastaların ise %44.3'sinde (23 hasta) etyolojide safra yolu hastalıkları mevcuttu ve alkol ise sırasıyla %14 ve %17.4 olguda sorumlu faktör idi. ‹lk başvurudaki APACHE II skoru UPMC hastalarında ortalama 11.4±5.9 iken ANH'deki hastalarda bu oran 8.9±4.8 idi (p>0.05). Hastalara cerrahi tedavi uygulama oranı ANH'deki hastalarda %90 olduğu halde UPMC'deki hastalarda yalnızca %53.8 idi (p<0.05). En sık uygulanan cerrahi girişim her iki birimde de multiple debritman idi. Yoğun bakımda yattığı gün sayısı UPMC de 18.2± 22.5 gün, ANH'de ise 6.4± 8.5 gün idi (p<0.05). Mortalite açısından değerlendirildiğinde ANH'deki hastalarda istatistiksel olarak anlamlı derecede yüksekti (p<0.05). Sonuç: Bizim sonuçlarımız yoğun bakım ünitelerinde deneyimli ekibin yakin takipi ile akut pankreatitin tedavisinde daha iyi sonuçlar alinabilecegini desteklemektedir.

Abstract

Background/aims: Improvements in the management of acute pancreatitis have led to a reduction in mortality rates, particularly in specialized units where technical resources and experienced personnel are available. The aim of this retrospective study was to compare the outcome of patients with acute pancreatitis treated in the intensive care unit of Ankara Numune hospital (where medical care was provided by surgeons only) with that of patients treated at the University of Pittsburgh Medical Center (where medical care was provided by a multidiciplinary team that of physicians, including surgeons and intensive care specialists). Methods: Fifty Turkish and 52 American patients with acute pancreatitis were included. There was an etiology of gallstones in 35 (70%) Turkish and 23 (44.3%) American patients and alcohol in 14 % and 17.4% of Turkish and American patients respectively, APACHE II score was 11.4±5.9 in American and 8.9±4.8 in Turkish patients on admission (p>0.05). Results: 90% of Turkish patients and 53.8% of American patients were treated surgically (p<0.05). The most common surgical procedure was multiple debridement in both institutes. Length of stay in intensive care was 18.2± 22.5 days and 6.4± 8.5 days in American and Turkish patients respectively (p<0.05). Mortality rates were significantly higher in Turkish patients (42% vs 25%, p<0.05). Conclusion: Intensive therapy with close monitoring by an experienced team can improve the outcome of acute pancreatitis.
