Urinary and atypical intestinal schistosomiasis: Case report

Özet
Şistozomiazis özellikle tropikal ülkelerde 200 milyondan fazla insanı etkileyen parazitik bir hastalıktır. ‹nsanları en sık infekte eden şistozoma tipleri S. mansoni, S. hematobium ve S. japonicum'dur. S. hematobium, mesane venöz pleksusuna göç ederken, olgun S. mansoni ve S. japonicum barsakların venöz pleksusuna göç ederler ve yumurtalarını bu dokulara bırakarak semptomlara neden olurlar. Patoloji, yumurtaların barsak veya mesane duvarı içerisinde kalsifikasyonla sonuçlanan gecikmiş hücresel hipersensitivite reaksiyonunu başlatmasıyla oluşur. İnfestasyonun şiddetine göre nodüller veya polipler gelişir. Bu olgu sunumunda literatür bulgularıyla uyumlu olan tipik üriner şistozomiazis ile birlikte çift kontrastlı kolon grafisiyle tespit edilen ve bilgimiz dahilinde daha evvel bildirilmemiş olan atipik kalın barsak tutulum bulguları tartışılmıştır.
Abstract
Schistosomiasis is a major parasitic disease estimated to affect over 200 million people in tropical countries. The three common species of schistosoma that infect mean are S. mansoni, S. hematobium and S. japonicum. Mature S. mansoni and S. japonicum migrate to the venous plexus of the intestine, while S. hematobium migrates to the vesical plexus. The parasites then deposit their eggs in these tissues and cause symptoms. The pathology is that the ova initiate an immune delayed hypersensitivity cell mediated reaction, which results in calcification within the bladder or intestinal walls, thus forming nodules or polyps according to the severity of the infestation. In this case report, we discuss the typical findings of urinary schistosomiasis and atypical features of large bowel involvement detected by double contrast barium enema, which to the best of our knowledge has not previously been reported.
