Özet
Çalışmamızda siroza bağlı portal hipertansiyonlu olgularda intrapulmoner vasküler dilatasyon ve hepatopulmoner sendrom sıklığı, bu patolojiler ile bazı klinik ve laboratuar bulgular arasındaki ilişki ile tanı yöntemlerinden kontrast ekokardiyografi ve akciğer perfüzyon sintigrafisinin karşılaştırılması amaçlanmıştır. Yöntem: Kardiyo-pulmoner hastalığı olmayan 31 sirotik hastada ve 8 sağlıklı kontrol olgusunda pulmoner fonksiyon testleri, arteriyel kan gazları, ekokardiyografi ve pulmoner şant indexini saptamak amacıyla akciğer perfüzyon sintigrafisi yapılmıştır. Bulgular: Olgulardan 5 tanesinde (%16.13) dik pozisyonda hipoksi (PaO2 <70mmHg) saptanmış olup , bunlardan 1 olguda (%3.22) yatar pozisyonda da hipoksi bulunmuştur. Pulmoner şant indexi 31 olguda ortalama %17.77, kontrol grubunda ise %7.61 saptanmıştır (p<0.001). ekokardiyografi 31 olgunun 12'sinde (%38.7) pozitif bulunmuştur. Ekokardiyografi ile hipoksi arasında istatistiksel ilişki saptanmamıştır. Hipoksik olgulardan 2 tanesinde ekokardiyografi negatif bulunmuştur. Pulmoner şant indeksi ve kardiyak index hipoksisi olan olgularda anlamlı yüksek (p< 0.001, p<0.05) bulunmuş, hipoksi ile Child-Pugh's evresi arasında pozitif korelasyon saptanmıştır. Sonuç: Çalışmamızda hipoksisi olmayan olgularda da intrapulmoner vasküler dilatasyonun varlığı gösterilmiştir. Bulgularımız pulmoner şant indeksi yüksekliğinin hipoksiyle ilişkili olduğunu, pozitif ekokardiyografi bulgusunun ise hipoksiden bağımsız olarak intrapulmoner vasküler dilatasyon saptamada duyarlı bir yöntem olduğunu desteklemektedir. Ancak negatif ekokardiyografi bulgusu ve hipoksisi olan olgularda tanı akciğer sintigrafisi ile desteklenmelidir.
Abstract
This study was undertaken to determine the frequency of intrapulmonary vascular dilatation and hepatopulmonary syndrome and to compare the contrast-enhanced echocardiography and lung perfusion scan. Methods: The results of contrast-enhanced echocardiography, lung perfusion scan (to determine pulmonary shunt index), pulmonary function tests and arterial blood gases were evaluated in 31 cirrhotic patients and eight healthy controls. Results: One patient (3.22 %) was hypoxemic in the supine position while five patients (16.13 %) were hypoxemic in the standing position. The mean pulmonary shunt index was 17.77 % in 31 patients and 7.61 % in the control group (p<0.001). Contrast-enhanced echocardiography was positive in 12 of 31 patients (38.7 %). There was no significant relationship between positive contrast-enhanced echocardiography and hypoxemia. One hypoxemic patient had normal contrast-enhanced echocardiography while pulmonary shunt index and cardiac index levels were significantly. There was a correlation between hypoxemia and Child-Pugh's stage. Conclusion: The existence of intrapulmonary vascular dilatation was demonstrated in normoxemic patients. These findings show that high pulmonary shunt index is related with hypoxemia and that positive contrast-enhanced echocardiography is sensitive in determining intrapulmonary vascular dilatation, independent of hypoxemia. However, the diagnosis should be confirmed by lung scanning in hypoxemic patients with no evidence of a shunt in contrast-enhanced echocardiography.
