Rektal şant indeksi ile portal hipertansiyon komplikasyonları arasındaki ilişkiler

Özet
Portal Hipertansiyonda (PHT) inferior mezente-rik ven düzeyindeki kollateraller hakkında bilgi veren rektal şant indeksi (RŞİ) nin son yıllarda kronik karaci¬ğer parankim hastalıklarının takibi ve özellikle özofa-gus varis kanamaları (ÖVK) hakkında öncü bilgiler ve¬receği belirtilmektedir. Çalışmamızda siroza bağlı PHT gelişmiş 41 olgumuzda RŞİ ile Chüd-Pugh's evre (C-P) ve PHT komplikasyonları arasındaki ilişkileri araştır¬mayı amaçladık. Sintigrafik olarak hastalara rektum üst kesimine 370 MBq Tc-99m perteknetat sonda ile ve¬rildikten sonra karaciğer ve kalp üzerine ilgi alanları 5 saniyelik aralarla 25 dakika süreyle bilgisayara kayde¬dilerek zaman-aktivite eğrileri elde edilip 30. saniyedeki eğri alanlarından RŞİ hesaplandı. 10 olgudan oluşan kontrol grubumuzda ortalama RŞİ %8.4±2 iken hasta grubumuzda ortalama RŞİ %67.21±3.1 olarak saptan¬dı. C-P evre A'da ortalama RŞİ %65.82±6.60; B'de %67.93+3.7 ve C'de %75.7+5.63 olarak saptanmış ve aradaki fark istatistiksel olarak anlamlı bulunmamış¬tır. Özofagus varisi(ÖV) olmayan hastalarda ortalama RŞl'i %62.7±5.97; 3. derece varisi olanlarda %70.85+3.81; ÖVK geçirmeyen olgularda %65.9±3.4; ÖVK öyküsü verenlerde ise %68.64±5.27 olarak saptan¬mış ve aradaki farklılıklar istatistiksel olarak anlamlı bulunmamıştır. Ancak ÖV olmayan ve 1. derece varisi olan olgu grubundaki RŞİ'nin 2 ve 3. derece ÖV olan olgu grubuna göre anlamlı düzeyde düşük olduğu sap¬tandı (p<0.05). İstatistiksel olarak anlamlı farklılık ol¬masa da asiti olan olgularımızın RŞl'i (%68.65+3.2) ol¬mayanlara göre (%64.11+4.75) göre daha yüksekti. Hepatik ensefalopatisi (HE) öyküsü olmayan hastalar¬da ort RŞÎ'i %62.41±3.58; HE öyküsü olanlarda ise %74.79+3.88 olarak saptanmış aradaki fark istatistik¬sel olarak anlamlı bulunmuştur. Ayrıca RŞİ ile total globulin, Ig G ve A düzeyleri arasında da istatistiksel olarak anlamlı ilişki saptandı. RŞİ'nin %70'in üzerinde olduğu olgularımızda prognozu olumsuz yönde etkile¬yen komplikasyonların sık olduğu gözlenmiştir. Çalış¬mamızda PHT'lu hastaların takibinde ve komplikas¬yonların gelişim riskinin değerlendirilmesinde RŞİ'nin önemli bir noninvasiv yöntem olduğu sonucuna varıl¬mıştır.
Abstract
It was reported in the recent years, the pa¬tients who have portal hypertension (PH); rectal shunt index (RSI) which determines the existence of collater¬al's at the inferior mesenteric vein level, gives predictive information about the progression of chronic liver par-ancimal diseases and particularly in oesophagus vari-ceal bleeding (BEV). In our study we aimed to investi¬gate the relations between RSI and Child-Pugh's (C-P) classification and PH complications in 41 patients who have cirrhosis and PH. Regarding the scintigraphic method, after giving 370 MBq Tc99m pertechnetat to the upper portion of rectum with a rectal tube, we calcu¬lated RSI in the 30th second by recording the regions of interest (RÖI) in the liver and the heart with 5 second, intervals lasting for 25 minutes and obtaining time-activity curves. In the control group including 10 sub¬jects in the mean RSI was 84+2% while in the patients group it was determined to be 67.21±3.1%. It was found out that in the patients with C-P grade A mean RSI was 65.82+6.6%, in grade B it was 67.93+3.7 and in grade C it was 75.7±5.63%, but these values had no statistical significance. Mean RSI of the patients without oesopha-geal varicies (OV) was 62.7±5.97% and it was 70.85+3.81% in the patients with 3rd degree OV, 62.7±5.97% in the subjects without BOV, and 68.64±5.27% in he subjects who had the history of BOV. The differences did not have statistical significances. On the other hand mean RSI was noticeably lower in the subjects without OV and with the first degree OV re¬specting the ones with the 2nd and the 3rd degree OV (p<0.05). Even though there was no statistical signifi¬cance, patients with ascites had a higher mean RSI (68.65±3.2%) than the patients without it (64.11+4.75%). The mean RSI of the patients with he¬patic encephalopathy (HE) was 74.79±3.88%. The mean RSI of the patients who had no HE was (62.41±3.58%) and the difference was determined to have statistical significance. In addition, a statistical significant rela¬tion was determined between RSI and the blood levels of total globulin, IgG and IgA. We also observed that the complications which worsen the prognosis was more fre¬quent in the patients with RSI higher than 70%. In our study, we concluded that during the follow up of the patients with PH, RSI is a significant and non-invasive method for evaluating and predicting the com¬plications.
