Özet

Kronik viral hepatitlerde başarılı bir tedavi için ge¬reken şartlar; viral replikasyonun kalıcı olarak durdu¬rulması ve karaciğerdeki nekroinflamatuar değişiklikle¬rin önemli ölçüde azaltılmasıdır. Bu prospektif çalışmada, interferon alfa-2a'nın HBV ve HCV'e bağlı olan etkilerini araştırmayı amaçladık: Çalışmaya histopatolojik olarak kronik aktif hepatit ta¬nısı konan yaş ortalaması 44±6 (18-62) yıl olan toplam 19 hasta alındı. Etyolojik ajan olguların 9'unda HBV ve 10'unda HCV idi. Tedavi programına alınan olgulara 6 ay interferon alfa-2a haftada 3 kez 4.5 milyon ünite sub-kütan olarak uygulandı. Tedavi başlangıcında ve altıncı ayın sonunda karaciğer biyopsisi tekrarlandı ve aynı pa¬tolog tarafından değerlendirilerek histolojik aktivite indeksi (HAI) hesaplandı. HAI tedavi öncesi ve sonrası sırası ile HBV grubunda 12.50+2.56, 10.50+4.47 ve HCV grubunda 11.78±3.19, 10.44+2.70 (p>0.05) idi. Karaci¬ğer biyopsi örneklerinde; portal inflamasyon, periportal nekroz, intralobuler dejenerasyon ve fibrozis her biri ay-ra ayrı değerlendirildiğinde tedavi öncesi ve sonrasında erken dönemde istatistiksel anlamlı fark saptanamadı. İnterferon alfa-2a'nın haftada üç kez 4.5 milyon ünite dozunda kullanılmasının krınik viral hepatitlerin tedavisinde karaciğerdeki histopatolojik. düzelme üzerine erken dönemde ancak kısmi olumlu bir etkisinin olduğu söylenebilir.

Abstract

The goal of management in the chronic viral hepatitis is to eradicate viral replication and ameliorate the necroinflammatory changes in the liver. In this prospective study, we investigated first to the ef¬fectiveness of interferon alpha-2a in eradicating HBV and HCV and second to see the histological outcome af¬ter therapy. Interferon alpha-2a given at a dosage of 4.5 MU three ti¬mes weekly for 6 months in 19 (pts). The cause of the chronic hepatitis is HBV in 9 pts. and HCV in 10 pts. All patients were performed liver biopsy pre and post treat¬ments and calculated histological activity index (HAI). Biopsies were compared by the same pathologist without knowledge of serological status and therapy. HAI is fo¬und 12.50+2.56, 10.50+4.47 in HBV group and 11.78±3.19,10.44+2.70 in HCV group pre and post treat¬ment, respectively. In liver biopsies; portal inflammation, periportal necrosis, interlobular degeneration and fibro-sis were evaluated one by one. There were no significant differences in this parameters between pre and post tre¬atment in liver biopsies at early stage. We conclude that, interferon alpha-2a given at a dosage of 4.5 MU three times weekly for 6 months has incomple¬te effectiveness on histological recovery in early phase.
