Özet

Bu çalışmada tıkanma sarılığının ayırıcı tanısı ve teda¬ visinin yönlendirilmesinde koledokoskopinin rolü araştırılmış¬ tır. Otuzüçü kadın 27' si erkek olan toplam 60 hasta kesin tanı¬ ya yönelik olarak preoperatif dönemde geniş bir spektrumda in¬ celendiler. Olgular koledokoskopinin tanıyı oluşturmadaki et¬ kinliği, preoperatuar incelemeler ile uyumu, mortalite ve mor- biditeye etkisi açısından değerlendirildiler. Hastalar koledo¬ koskopi yardımı ile elde olunan intraoperatif tanıları ile pre¬ operatif tanıları kıyaslanarak üç grupta toplandılar. Otuzbeş hastada (%58.3) koledokoskopik inceleme preoperatif tanıyı doğrulamış, 12 hastada (%20) tıkanma sarılığının kesin tanısı koledokoskopi aracılığıyla belirlenmiş, onüç hastada (%21.7) ise koledokoskopik inceleme preoperatif tanıyı anlamlı olarak değiştirmiştir. Sonuç olarak intraoperatif "fleksibl" koledokos¬ kopi özellikle dış safra yolları patolojilerine bağlı tıkanma sarılıklı hastalarda kesin tanı, cerrahi planlama ve ameliyat tipinin belirlenmesinde çok yararlı bir tanı aracı olarak belir¬ miştir.

Abstract

The aim of this study is to evaluate the role of cho¬ledochoscopy in the differential diagnosis and management of obstructive jaundice. A total of 60 patients (33 female and 27 male) were evaluated for the accuracy of preoperative diagnos¬tic methods, the effectiveness of choledochoscopy in the estab¬lishment of the diagnosis, the comparison of intraoperative ver¬sus preoperative diagnosis and morbidity dnd mortality rela¬ted to the procedure. In 35 patients (58.3 %) choledochoscopy confirmed the preoperative diagnosis. In twelve patients (20 %) the exact diagnosis of obstructive jaundice was established by choledochoscopy and the preoperative diagnosis changed signi¬ficantly after choledochoscopy in thirtheen patients (21.7 %). Intraoperative flexible choledochoscopy appeared as a very use¬ful diagnostic tool in the diagnosis, surgical decision making and choice of the operation in patients with obstructive jaun¬dice.
