Özet

Helicobacter^ pylori (H. pylori) infeksiy]onu, dünya¬daki en yaygın infeksiy ondur. Gelişmiş ülkelerde sani-tasyon sorunu çözüldüğü için, H. pylori infeksiyonu ye^ ni kuşaklarda azalmıştır. Gelişmekte olan ülkelerde ise infeksiycM, ilk yıllarda alınmakta ve hayat boyu devam etmektedir. Bugünkü düşünceye göre duodenal ülser, bulbustaki gastrik metaplazi üzerine yapışan H. pylori ' nin neden olduğu inflamasyona bağlıdır. Bu çalışmanın amacı, Türkiye'de H. pylori 'nin ve bulber gastrik metap-lazinin yaş gruplarına göre dağılımını'incelemektir. Bu amaçla polikliniğimize başvuran 319 kişi çalışmaya alındı. Korpustan iki, antrumdan iki ve bulbustan dört adet endoskopik biyopsi alındı. Biyopsi spesmenleri gastrik metaplaziyi, mukozal inflamasyonu ve H. pylori 'yi göstermek için sırayla periodic acid-Schiff, hematoxy-lin-eosin, ve War thin-Starry ile boyandı. Sonuçlar çap¬raz korelasyon yöntemiyle ve student's t testiyle karşılaş¬tırıldı. Gastrik H. pylori prevalansının 49 yaşına kadar benzer olduğu, 40-49 yaş grubunda pik yaptığı ve sonra anlamlı şekilde azaldığı izlendi (p<0.001). Gastrik me¬taplazi ve bulber H. pylori prevalansında ise anlamlı bir fark saptanmadı. 10-49 ve 50-90 yaş grupları da in¬celendi. E/K oranı, aktif gastrit ve duodenit, atrofik gastrit, duodenal ülser, bulber H. pylori ve gastrik me¬taplazi oranları her iki grupta da benzerdi. NSAID alı¬mı ve intestinal metaplazi oranları ikinci grupta yüksek bulundu (p<0.05). 50 yaş ve üstünde gastrik H. pylori prevalansının düşük olması, bu grupta intestinal metaplazi ve NSAID kullanım oranlarının yüksek ol¬masına bağlandı.

Abstract

Infection of Helicobacter pylori (H. pylori) is the most widespread infection on the World. H. pylori in¬fection has diminished in younger population in develo¬ped countries, presumably depending on sanitation and hygiene practices. However, infection has been taken early and has persisted for the life in the developing co¬untries. The postulate is that the duodenal ulcer is the re¬sult of intense inflammation caused by bacteria at the si¬te of gastric metaplasia. The aim of this study is to inves¬tigate the distribution of gastric metaplasia and H. pylori related to ages in Turkey. 319 persons admitted to our department were studied. Multiple endoscopic biop¬sies were obtained from the gastric corpus, antrum and the duodenal bulb of each subject. Sections of biopsy tis¬sue were stained with Periodic Acid-Schiff, Hematoxylin-eosin, and Warthin-Starry to examine for gastric metap¬lasia, mucosal inflammation, and H. pylori, respectively. The results were compared with correlation matrix and student's t test. It was seen that the prevalence of gastric H. pylori seems the same until 49 years, reaches to top between 40 and 49 years, and significantly diminish af¬ter that (p<0.001). But, there was no significant differen¬ce for prevalence of gastric metaplasia and bulber H. pylori. Groups of 10-49 and 50-90 years also evaluated. Male I female ratio, rates of active gastritis, active duode¬nitis, atrophic gastritis, duodenal ulcer, duodenal bulb gastric metaplasia and bulber H. pylori were similar in both groups. NSAID use and rate of intestinal metapla¬sia were higher in later group (p<0.05). The lower preva¬lence of gastric H. pylori in later group was attributed to higher rates of intestinal metaplasia and NSAID use.
