Ekstrahepatik maligniteli hastalarda HBsAg / anti-HCV seropozitifliği ve risk faktörleri ile ilişkisi

Özet

Bu çalışma, primer karaciğer tümörü dışında malignitesi olan hastalarda HBsAg ve anti-HCV pozitifliği sıklığı ile risk faktörleri arasındaki ilişkiyi araştırmak için yapıldı. Çalışmaya doku tanısı olan 98 ekstrahepatik maligniteli hasta (56 kadın, 42 erkek, yaş ortalaması 52.1± 13.7) ve kontrol grubu olarak, malignite dışı nedenlerle hastaneye başvuran ve malignite saptanmayan 76 hasta (49 kadın, 27 erkek, yaş ortalaması 54.0 ± 16.4) alındı. Malign grubun % 84'ünün solid, % l6sının ise hematopoetik tümörü vardı. Malign grupta kan transfüzyonu ve operasyon öyküleri kontrol grubuna göre anlamlı olarak yüksekti (sırasıyla % 61'e karşı % 24, OR: 5.0 [2.4 0.05). Malignite grubunda bulaş faktörlerinden yalnızca kan transfüzyonu öyküsü olanlarda HBsAg pozitifliği oranı (% 18.7), kan transfüzyonu öyküsü olmayanlara göre (% 0) anlamlı olarak daha yüksek bulundu (p<0.05); bununla birlikte lojistik regresyon analizi ile yapılan değerlendirmede, malignite grubundaki deneklerde HBsAg pozitiflik olasılığını kestirmede bağımsız olarak etkili olan herhangi bir risk faktörü ayırt edilmedi. Sonuç olarak, malign hastalarda kontrol grubuna göre kan transfüzyonu ve operasyon öyküsü daha yüksekti. HBsAg-pozitif malign olgularda kan transfüzyonu sıklığı daha yüksek oranda bulunmasına rağmen, risk faktörleri bağımsız olarak HBsAg pozitiflik olasılığını kestirmede etkili bulunmadı ve araştırılan diğer risk faktörlerinden etkilendiği saptandı.

Abstract

The aim of this study was to investigate the frequency of HBsAg/anti-HCV seropositivity and its association with risk factors in patients with extrahepatic malignancies. Ninety-eight patients diagnosed extrahepatic malignancies (56 female, 42 male, mean age: 52.1±13.7) and, as a control group, 76 patients who had no detected malignancy (49 female, 27 men, mean age: 54.0 ± 16.4) were included in the study. Eighty-four percent of malign group had solid tumor and in 16 % had hematopoetic tumor. The histories of blood transfusion and operation were significantly higher in the malign group comparing to controls (61% vs. 24% , OR: 5.0 [2.4<OR<10.3], p<0.05 and 83% vs. 55%, OR: 3.84 [1.83<OR<8.18], p<0.05; respectively). HBsAg seropositivity rates were significantly higher in the malign group (12.2%) than in the control group (1.3%) [OR: 10.47, 1.36<OR<220.46; p<0.05]. Although anti-HCV prevalence was higher in the malign group (5.1% vs. 2.6%), this difference was not statistically significant (p>0.05). In the malign group, among risk factors, only the history of blood transfusion significantly increased HBsAg seropositivity rates (18.7% in transfused vs. 0% in non-transfused, p<0.05). However, logistic regression analysis showed that there were no independent risk factors to predict the probability of HBsAg seropositivity in those with malignancy. We conclude that although the frequency of blood transfusion and operation were higher in the malign group than in controls and, in HBsAg positive patients with malignancy, the frequency of blood transfusion was higher when compared to that of HBsAg-negative patients with malignancy, these risk factors are not independent factors to predict the probability of HBsAg seropositivity, and they are affected by other risk factors examined for this study.
